中医治疗葡萄膜炎研究进展

摘　　要：葡萄膜炎是临床常见的一种眼部炎症疾病,包括葡萄膜、视网膜、视网膜血管、玻璃体等炎症,葡萄膜富含色素和血管,有营养眼球的作用,且血流缓慢,许多疾病可通过血流影响葡萄膜而使其致病。在中医领域按其发病部位及病症特点,分为"瞳神紧小""瞳神干缺""云雾移睛""视瞻昏渺""狐惑病"等。葡萄膜炎好发于青壮年,病因复杂,复发率高,治疗不当可导致视功能减退、视力严重下降、甚至失明,且致盲率为1.1%~9.2%。近年来中医传统治疗方法结合西医常规治疗常用于葡萄膜炎的临床治疗,临床疗效明显优于单纯西医治疗,并具有减轻西药不良反应,降低复发率,提高免疫力,缩短疗程等作用。中医认为葡萄膜炎与肝、肾两脏密切相关。通过对近年来结合治疗葡萄膜炎的临床观察及相关实验的文献概况,从病因病机、辨证论治、分期治疗(急性期、慢性期)、复发与并发症的治疗、常用方剂研究(龙胆泻肝汤、祛风活血丸)及中医外治法(针刺、穴位埋线)等几个方面进行总结,为葡萄膜炎的中医治疗提供参考。Uveitis is a common clinical inflammatory disease of the eye,including inflammation of the uvea,retina,retinal blood vessels,vitreous,etc. Uveitis is rich in pigments and blood vessels,and has the effect of nourishing the eyeball,and the blood flow is slow. Many systemic diseases can affect the uvea and cause the disease. In traditional Chinese medicine,according to the location of the disease and the characteristics of the disease,it can be divided into " Tongshen Jinxiao" " Tonshen Gaque" " Yunwu Yijing" " Shizhan Yunmiao" " Huhuo Disease" and so on. Uveitis occurs to young and middle-aged adults. The cause is complex and the recurrence rate is high. Improper treatment can lead to decreased visual function,severely reduced vision,and even blindness. The blindness rate is 1. 1% ~ 9. 2%. In recent years,traditional Chinese medicine treatment methods combined with conventional western medicine treatment methods are often used in the clinical treatment of uveitis. The clinical efficacy is significantly better than simple western medicine treatment,and it has the advantages of reducing the toxic and side effects of western medicine,reducing the recurrence rate,improving immunity,and shortening the course of disease. However,due to its complicated etiology,the pathogenesis is not fully understood,but traditional Chinese medicine believes that uveitis is closely related to liver and kidney. Through the clinical observation and literature of relevant experiments on the combination of traditional Chinese medicine and western medicine in the treatment of uveitis in recent years,from the etiology and pathogenesis,syndrome differentiation and treatment,staged treatment( acute and chronic),relapse and complications treatment,commonly-used prescription research( Longdan Xiegan decoction,Qufeng Huoxue Pills)and external treatment of traditional Chinese medicine( acupuncture,acupoint embedding) were summarized to provide a reference for the treatment of uveitis with traditional Chinese medicine.
