摘  要:目的观察超声光散射成像(DOT)结合PET-CT诊断乳腺癌的价值。方法选择2017年5月至2019年5月在枣矿集团中心医院行乳腺病变切除活检的女性患者42例,共45个乳腺病灶,均经病理检查证实,患者术前接受DOT和PET-CT检查,测量乳腺病灶的总血红蛋白浓度(THC)及病灶最大标准摄取值(SUVmax)。以病理诊断为"金标准",绘制受试者工作特征曲线(ROC),计算曲线下面积(AUC)值分析DOT与PET-CT及其联合诊断乳腺癌的临床价值。以Pearson相关及散点图分析THC与SUVmax之间的关系。结果 42例患者45个病灶中,良性病灶20个,病灶直径1.2~6.8cm,恶性病灶25个,病灶直径0.8~5.5cm;良性组THC与SUVmax均显著低于恶性组,差异有统计学意义(P<0.05);以THC>170.88μmoL/L作为诊断恶性的截断值,诊断乳腺癌的敏感度、特异度分别为65.7%、85.0%,AUC值为0.776;以SUVmax>3.28作为诊断恶性的截断值,诊断乳腺癌的敏感度、特异度分别为82.9%、75.0%,AUC值为0.835;二者联合诊断的AUC值最大(0.873),敏感度、特异度分别为85.7%、75.0%;Pearson相关分析显示,THC与SUVmax显著正相关(r=0.609,P<0.05)。结论 DOT与PET-CT均为乳腺癌的有效诊断方法,二者联合应用具有更高的诊断价值。Objective To observe the value of ultrasound diffused optical tomography(DOT)combined with PET-CT in the diagnosis of breast cancer.Methods 42 female patients undergoing resection and biopsy of breast lesions in the Central Hospital of Zao Mining Group from May 2017 to May 2019 were selected,with a total of 45 breast lesions confirmed by pathological examination.The patients were examined with DOT and PET-CT before operation.The total hemoglobin concentration(THC)of the breast lesion and the maximum standard uptake value(SUVmax)of the lesion were measured.With pathological diagnosis as the golden standard,the receiver operating characteristic(ROC)curve was drawn.The area under the curve(AUC)was calculated to analyze the clinical value of DOT and PET-CT and their combined diagnosis for breast cancer.The relationship between THC and SUVmax was analyzed by Pearson correlation analysis and scatter diagram.Results In the 42 patients,there were 20 benign lesions in the 45 lesions.The diameter of the lesion was 1.2-6.8cm.There were 25 malignant lesions,and the diameter of the lesion was 0.8-5.5cm.THC and SUVmax of the benign group were significantly lower than those in the malignant group(P<0.05).With THC>170.88μmol/L as the diagnostic cutoff value,the sensitivity and specificity for diagnosis of breast cancer were 65.7%and 85.0%,and the AUC was 0.776.With SUVmax>3.28 as the diagnostic cutoff value,the sensitivity,and specificity for diagnosis of breast cancer were 82.9%and 75.0%,and the AUC was 0.835.The AUC of combined diagnosis of the two was the largest(0.873).The sensitivity and specificity were 85.7%and 75.0%.Pearson correlation analysis showed that THC was significantly positively correlated with SUVmax(r=0.609,P<0.05).Conclusion DOT and PET-CT are effective diagnostic methods for breast cancer,and their combined application has higher diagnostic value.
