摘  要:慢性阻塞性肺疾病(chronic obstructive pulmonary disease,COPD)病位在肺,与脾、肾密切相关,日久及心、脾、肾,肺脾肾俱虚,脾虚失运,则见膈肌疲劳、痰饮内生。膈肌疲劳与肺、脾、肾关系密切,三者生理功能上相互为用,使得脾土化生有源,肺金宣肃有权,肾水摄纳有根,共同促进加强机体的呼吸能力。久病肺脾虚损,宗气不足,呼吸泵动力不足,肺不主气则见喘息。故治疗COPD膈肌疲劳应以“肺本虚”为核心,通过补益肺、脾、肾以升补宗气,以宗气带动周身气血运行,五脏六腑之气得以循环则病自缓。Chronic obstructive pulmonary disease(COPD) is located in the lung,which is closely related to the spleen and kidney and involves the heart,spleen and kidney in the long run.If the lung,spleen and kidney are all deficient and the spleen is not functioning,the diaphragmatic fatigue and phlegm retention will appear.Diaphragmatic fatigue is closely related to the lung,spleen and kidney.The three physiologically interact with each other,making the spleen have resource to generate,the lung have ability to ventilate,the kidney have the root to absorb the water,and the respiratory function normal.Long-time spleen deficiency,pectoral qi insufficiency and insufficient power of respiratory pump result in the lung failing to govern qi,and dyspnea.Therefore,the treatment of diaphragmatic fatigue in COPD should focus on "deficiency of the lung itself".By tonifying the lung,spleen and kidney,pectoral qi can be nourished to promote the circulation of qi and blood in the whole body.The smooth circulation of qi in the five zang-organs and six fu-organs can relieve the disease.
