膝关节镜手术联合针刀治疗早中期膝骨关节炎合并鹅足滑囊炎疗效观察

摘　　要：目的:观察膝关节镜手术联合针刀治疗早中期膝骨关节炎合并鹅足滑囊炎的临床疗效。方法:选取2018年1月至2019年12月北京中医药大学第三附属医院治疗的早中期膝骨关节炎合并鹅足滑囊炎患者80例,随机分为治疗组和对照组,每组40例。两组患者均给予膝关节镜手术治疗,术后第3天,治疗组针对鹅足处给予针刀治疗,对照组给予布洛芬缓释胶囊口服联合中药热敷治疗。采用Lysholm评分、视觉模拟评分(visual analogue score,VAS)评估治疗前后膝关节功能和鹅足处疼痛,并记录不良反应。结果:治疗组治疗后1周、3周、8周Lysholm评分均高于对照组,差异有统计学意义(P<0.05);治疗组治疗后1周、3周、8周VAS评分均低于对照组,差异有统计学意义(P<0.05)。结论:关节镜联合针刀治疗早中期膝骨关节炎合并鹅足滑囊炎,能改善患者膝关节功能,缓解鹅足处疼痛,有助于术后膝关节功能恢复。Objective:To observe the clinical curative effect of knee arthroscopic surgery combined with acupotomy on early and middle stage knee osteoarthritis combined with goose foot bursitis.Methods:A total of 80 patients with early and middle stage knee osteoarthritis complicated with goose foot bursitis treated in the Beijing University of Chinese Medicine Third Affiliated Hospital from January 2018 to December 2019 were selected and randomly divided into the treatment group and the control group,with 40 cases in each group.Both groups were treated with arthroscopic surgery,on the third day after operation,the treatment group was additionally given acupotomy treatment for goose feet,and the control group was additionally given Ibuprofen Sustained Release Capsule combined with hot compress of Chinese medicinals.Lysholm score and visual analogue score(VAS) were used to evaluate knee function and goose foot pain before and after treatment,and adverse reactions were recorded.Results:The Lysholm score of the treatment group was higher than that of the control group on the 1 st week,3 rd week and 8 th week after treatment,and all the differences were statistically significant(P<0.05).The VAS scores of the treatment group on the 1st week,3 rd week and 8th week after treatment were lower than those of the control group,and all the differences were statistically significant(P<0.05).Conclusion:In terms of early and middle stage knee osteoarthritis combined with goose foot bursitis,knee arthroscopic surgery combined with acupotomy can improve the function of knee joint,relieve the pain of goose foot,and help to recover the function of knee joint after operation.
