摘　　要：目的:观察电针对颅脑外伤昏迷患者大脑中线及环池的影响。方法:选取重度颅脑损伤昏迷患者60例,按照随机数字表法分为对照组及电针组,每组30例。对照组给予常规治疗,电针组在对照组治疗的基础上加用电针治疗。观察两组患者治疗前后格拉斯哥昏迷评分(glasgow coma scale,GCS)变化情况,对比两组治疗后1个月格拉斯哥预后分级(glasgow outcome score,GOS)、CT下大脑中线移位及环池形态变化情况。结果:两组患者治疗后、治疗后1个月GCS评分均高于治疗前(P<0.05),且电针组高于对照组,差异有统计学意义(P<0.05)。电针组治疗后1个月GCS评分、GOS分级优于对照组,差异有统计学意义(P<0.05)。运用CT行颅脑扫描示,中线移位>10mm者,电针组少于对照组,环池宽度>3mm者,电针组多于对照组,差异有统计学意义(P<0.05)。结论:电针治疗可提高颅脑外伤昏迷患者GCS评分,改善GOS分级,减轻患者继发性脑损伤,其机制可能与促进患者大脑中线移位及环池形态恢复相关。Objective:To observe the influence of electroacupuncture on brain midline and cistern ambiens of coma patients after craniocerebral trauma.Methods:A total of 60 patients with severe coma after craniocerebral trauma were randomly divided into the control group and the electroacupuncture group,with 30 cases in each group.The control group was given conventional treatment,and the electroacupuncture group was given electroacupuncture on the basis of the control group.The change of glasgow coma scale(GCS) before and after treatment in the two groups were observed,and glasgow outcome score(GOS),brain midline displacement under CT and morphological changes of cisterna ambiens cistern were compared between the two groups one month after treatment.Results:The GCS scores of the two groups after treatment and one month after treatment were higher than those before treatment.The difference was statistically significant(P<0.05).One month after treatment,the GCS score and GOS grade of the electroacupuncture group were better than those of the control group,and all the differences were statistically significant(P<0.05).Brain scanning with CT showed that the patients with midline displacement more than 10 mm in the electroacupuncture group were less than those in the control group,while the patients with annular cisterna ambiens width more than 3 mm in the electroacupuncture group were more than those in the control group,and all the differences were statistically significant(P<0.05).Conclusion:In terms of coma after craniocerebral trauma,electroacupuncture can improve GCS score and GOS grade,and alleviate secondary brain injury.The mechanism may be related to the promotion of midline displacement and the recovery of cisterna ambiens morphology.
