高血压前期人群中医体质分布特点及与血尿酸水平的相关性研究

摘　　要：目的探讨高血压前期人群中医体质分布特点及与血尿酸水平的相关性。方法纳入2018年12月—2019年10月在广西中医药大学附属瑞康医院及广西贵港市覃塘区第二人民医院体检符合《中国高血压防治指南》中关于高血压前期诊断标准者292例,记录患者9种中医体质类型占比及血尿酸水平、舌脉象特点,Logistics回归分析中医体质和血尿酸水平的关系,比较与血尿酸水平相关的中医体质类型患者间一般资料、肝肾功能及血脂、血糖水平的差异。结果292例高血压前期患者中,体质分布前三的是痰湿质[25.0%(73/292)]、阴虚质[19.5%(57/292)]、阳虚质[18.2%(53/292)],脉象分布前三的是细脉[26.4%(77/292)],滑脉[25.7%(75/292)]、沉脉[25.3%(74/292)],舌色分布前三的是淡红舌[45.5%(133/292)]、淡白舌[30.8%(90/292)]、红舌[13.7%(40/292)],苔质分布前三的是薄苔[36.0%(105/292)]、厚苔[25.3%(74/292)]、厚腻苔[12.0%(35/292)]。Logistics回归分析显示平和质、痰湿质、湿热质与血尿酸水平密切相关(OR=0.242,1.894,2.602,P均<0.05)。平和质、痰湿质、湿热质患者间腰围、臀围及肌酐、血尿酸、三酰甘油、低密度脂蛋白胆固醇、血糖水平比较差异均有统计学意义(P均<0.05),体质指数及谷丙转氨酶、总胆红素、高密度脂蛋白胆固醇、总胆固醇水平比较差异均无统计学意义(P均>0.05)。结论高血压前期人群中以痰湿质、阴虚质、阳虚质占比多,痰湿质、湿热质与血尿酸水平升高密切相关,不同中医体质和心血管疾病的危险因素之间显著相关。Objective It is to explore the characteristics of traditional Chinese medicine constitution distribution of prehypertensive population and its correlation with blood uric acid level.Methods 292 patients who received medical examination in Ruikang Hospital Affiliated to Guangxi University of Traditional Chinese Medicine and the Second People’s Hospital of Qintang District from December 2018 to October 2019 and met the prehypertension diagnostic criteria in the“Guidelines for the Prevention and Treatment of Hypertension in China”were included.The proportions of the 9 kinds of constitution types,uric acid levels,tongue and pulse characteristics of the patients were recorded,the relationship between TCM constitution and the levels of blood uric acid was analyzed by logistic regression analysis,and the differences in general data,liver and kidney functions,the levels of blood lipids and blood glucose among the patients with TCM constitution related to blood uric acid levels were compared.Results Among the 292 patients with prehypertension,the top three constitution distributions were phlegm-dampness constitution[25.0%(73/292)],Yin-deficiency constitution[19.5%(57/292)],and Yang-deficiency[18.2%(53/292)],the top three pulse condition were feeble pulse[26.4%(77/292)],slippery pulse[25.7%(75/292)],deep pulse[25.3%(74/292)],the top three distribution of tongue color were light red tongue[45.5%(133/292)],light white tongue[30.8%(90/292)]and red tongue[13.7%(40/292)],the top three distribution of tongue furs were thin fur[36.0%(105/292)],thick moss[25.3%(74/292)],thick greasy fur[12.0%(35/292)].Logistics regression analysis showed that the normal constitution phlegm-dampness constitution,dampness-heat constitution were closely related to blood uric acid level(OR=0.242,1.894,2.602,all P<0.05).There were statistically significant differences in waist circumference,hip circumference,the levels of creatinine,blood uric acid,triacylglycerol,low-density lipoprotein cholesterol,and blood sugar among patients with normal cons
