痛经伴发痤疮病机治法探讨

摘　　要：痛经虽与痤疮分属不同疾病,但二者病因病机却有相似之处,气滞、血瘀、寒凝、湿热等病理产物不仅可以瘀阻胞宫经脉致使“不通则痛”,亦可诱导痤疮的发生。如气滞血瘀型痛经,日久化火上炎,痤疮丘疹色红,皮损经前加重;湿热蕴结型痛经,热盛肉腐成脓,痤疮囊肿脓疱,面部油腻痒痛;寒热错杂型痛经,少腹冷痛喜暖,目赤咽干面红,粉刺丘疹黑头。故临证时应异病同治,随证变法,施以疏肝理气除郁热、清热利湿化腐脓、清上温下调寒热、火针刺疮消疡肿等法治疗。Although dysmenorrhea and acne are different,their etiology and pathogenesis are similar.Qi stagnation,blood stasis,cold coagulation,damp-heat and other pathological products can not only block the uterine meridians and cause“uncomfortable pain”,but also induce the occurrence of acne.For example,in dysmenorrhea with the syndrome of qi stagnation and blood stasis,the liver fi re fl ares up over time,the acne papules turn red,and the skin lesions aggravate before menstruation.While in dysmenorrhea with the syndrome of accumulation and binding of damp-heat,fl esh suppurates due to heat exuberance,acne cysts pustule,and the face gets greasy,itchy and aching.And in dysmenorrhea with the syndrome of intermingled cold and heat,the lower abdomen feels cold and painful and the patients prefer warm,with red eyes,dry throat,red face,and the blackhead acne papules.Therefore,diff erent diseases should be treated with the same method in clinical practice,and methods should be altered according to different syndromes,such as dispersing the liver and regulating qi to remove stagnation and heat,clearing heat and promoting diuresis to eliminate suppuration and pus,clearing upper jiao and warming the kidney yang to coordinate cold and heat,and using fi re needle to eliminate swollen sores,etc.
