血府逐瘀汤联合广谱抗生素治疗剖宫产术后血肿

摘　　要：目的观察血府逐瘀汤联合广谱抗生素治疗剖宫产术后血肿的临床效果。方法选择46例剖宫产术后盆腔血肿患者作为研究对象,采用随机数字表法分为对照组和观察组,各23例。对照组予广谱抗生素治疗,观察组在对照组的基础上予血府逐瘀汤内服外敷治疗。观察2组恢复正常时间、治疗前后白细胞、C反应蛋白(CRP)、降钙素原(PCT)、疼痛视觉模拟评分(VAS)水平。结果观察组体温恢复正常时间和血肿消失时间少于对照组(P<0.05)。治疗后1周、2周时观察组血肿大小均低于对照组(P<0.05)。治疗后2组白细胞、CRP、PCT水平均较治疗前显著降低(P<0.05),且观察组上述指标水平均低于对照组(P<0.05)。治疗1周、治疗2周时2组VAS均较治疗前显著降低(P<0.05),且观察组低于对照组(P<0.05)。结论血府逐瘀汤内服外敷联合广谱抗生素治疗剖宫产术后盆腔血肿能够降低白细胞、CRP和PCT水平,促使炎症恢复,缩短体温恢复时间,促进血肿吸收,减轻临床症状。Objective To observe the clinical effect of Xuefu Zhuyu Decoction combined with broad-spectrum antibiotics in the treatment of hematoma after cesarean section.Methods A total of 46 patients with pelvic hematoma after cesarean section were randomly divided into control group and observation group,with 23 cases in each group.The control group was treated with broad-spectrum antibiotics,and the observation group was treated with internal and external application of Xuefu Zhuyu Decoction on the basis of the control group.The recovery time,levels of white blood cells,C-reactive protein(CRP),procalcitonin(PCT)and visual analogue scale(VAS)of pain in the two groups were observed before and after treatment.Results The recovery time of body temperature and disappearance time of hematoma in the observation group were less than those in the control group(P<0.05).In the 1st week and 2nd week after treatment,the hematoma size of the observation group was smaller than that of the control group(P<0.05).After treatment,the levels of WBC,CRP and PCT in the two groups were significantly lower than those before treatment(P<0.05),and the above indexes in the observation group were lower than those in the control group(P<0.05).After 1 week and 2 weeks of treatment,the VAS of the two groups were signifi cantly lower than before treatment(P<0.05),and the observation group was lower than the control group(P<0.05).Conclusion The internal and external application of Xuefu Zhuyu Decoction combined with broad-spectrum antibiotics in the treatment of pelvic hematoma after cesarean section can reduce the levels of WBC,CRP and PCT,promote the recovery of infl ammation,shorten the recovery time of body temperature,promote the absorption of hematoma,and alleviate clinical symptoms.
