AMI患者血浆miR-221-3p、miR-208b-3p表达水平与心肌损伤标志物及左心功能的关系

摘　　要：目的研究急性心肌梗死(AMI)患者血浆微小MicroRNA(miR)-221-3p、miR-208b-3p表达与心肌损伤标志物及左心功能的关系。方法132例AMI患者为AMI组,另选取48例健康查体人群作为对照组。采用全球急性冠状动脉事件注册(GRACE)评分对AMI患者进行危险分层,采用实时荧光定量PCR法检测各组血浆miR-221-3p、miR-208b-3p表达水平,比较不同危险分层的AMI患者血浆miR-221-3p、miR-208b-3p表达水平,分析二者与心肌损伤标志物肌钙蛋白I(cTnI)、磷酸激酶同工酶(CK-MB)、内皮素1(ET-1)及左心功能参数左心室射血分数(LVEF)、左心室舒张末期容积(LVEDV)及左心室收缩末期容积(LVESV)的关系。结果与对照组比较,AMI组患者的血浆miR-208b-3p、miR-221-3p表达明显较高(P<0.05)。不同GRACE危险分层的AMI患者血浆miR-221-3p、miR-208b-3p表达水平差异有统计学意义(P<0.05)。高危组患者血浆miR-208b-3p、miR-221-3p明显高于中危及低危组(P<0.05)。AMI组患者血浆miR-221-3p、miR-208b-3p与cTnI呈正相关(r值分别为0.480、0.518,P<0.05),与LVEF呈负相关(r值分别为-0.535、-0.503,P<0.05),而与CK-MB、ET-1、LVEDV、LVESV无相关性(P>0.05)。结论AMI患者血浆miR-221-3p、miR-208b-3p表达水平升高,并且与心肌损伤严重程度及左心功能的有关,有可能成为新的AMI标志物。Objective To investigate the correlation between the expression levels of mir2213p,mir208b-3p and the markers of myocardial injury as well as left ventricular function in patients with acute myocardial infarction(AMI).Methods A total of patients with AMI were enrolled as AMI group,and 48 healthy subjects were enrolled as control group.The global acute coronary event registry(GRACE)score was used to stratify the risk of AMI.The expression levels of mir221-3p and mir208b 3p were detected by real time fluorescent quantitative PCR and compared.The correlation between mir2213p,mir208b 3p and cardiac troponin I(cTnI),phosphokinase isoenzyme(CKMB),endothelin 1(ET1),left ventricular ejection fraction(LVEF),left ventricular end diastolic volume(LVEDV),left ventricular end systolic volume(LVESV)was analyzed.Results Compared with those in control group,the expression levels of mir208b 3p and mir2213p in AMI group were significantly increased(P<0.05).There were significant differences in the expression levels of mir2213p and mir208b 3p in AMI patients with different risk stratification of GRACE(P<0.05).The plasma levels of mir208b 3p and mir2213p in high risk group were significantly higher than those in the other two groups(P<0.05).In AMI group,mir2213p and mir208b 3p were positively correlated with cTnI,which were negatively correlated with LVEF(P<0.05),however,which not correlated with CKMB,ET1,LVEDV and LVESV(P>0.05).Conclusion The expression levels of mir2213p and mir208b 3p in patients with AMI are increased and related to the severity of myocardial injury and left ventricular function,which may become a new marker of AMI.
