摘 要:目的研究COPD患者使用噻托溴铵与呼吸训练联合干预对患者肺功能、血清人克拉拉细胞蛋白(CC16)、可溶性E-选择素(sE-SLT)、淀粉样蛋白A(SAA)等影响。方法100例COPD患者分为对照组和研究组,每组50例。对照组使用噻托溴铵治疗并辅助以常规护理,研究组使用噻托溴铵治疗、常规护理并辅助以呼吸训练,2组对比治疗后不同时间段中肺功能情况[呼气峰流速(PEF)、1S用力呼气容积(FEV1)、最大肺活量(FVC)]、血清CC16、sE-SLT、SAA水平、不良反应率、生活质量评分、运动耐力6 min步行距离测验(6WMD)情况。结果研究组患者治疗后1、3、6个月后PEF、FEV1、FVC水平均高于对照组(P<0.05);研究组患者治疗1、3、6个月后血清CC16、sE-SLT、SAA水平均低于对照组(P<0.05);研究组患者治疗1、3、6个月后6WMD平均均高于对照组(P<0.05),治疗后不同时段内研究组不良反应率低于对照组,生活质量评分低于对照组(P<0.05)。结论COPD患者使用噻托溴铵治疗,接受呼吸训练干预,可有效促进肺功能的恢复,改善血清CC16、sE-SLT、SAA水平,提高运动耐力,降低不良反应,增强治疗效果及预后生活质量,该模式具有较高临床应用价值。Objective To investigate the effects of tiotropium bromide combined with respiratory training on the lung function and serum levels of CC16,sE-SLT and SAA in patients with chronic obstructive pulmonary disease(COPD).Methods One hundred patients with COPD who were treated in our hospital from January 2019 to April 2020 were ebrolled in the study,who were randomly divided into control group and research group,with 50 cases in each group,The patients in control group were treated by routine nursing,however,the patients in research group,on the basis of control group,were treated by thiamethoxam bromide combined with respiratory exercise.After treatment,the pulmonary function,serum CC16,sE-SLT,SAA levels,adverse reaction rate,quality of life score,sports endurance were observed and compared between the two groups.Results At 1m,3m,6m after treatment,the levels of PEF,FEV1 and FVC levels in research group were significantly higher than those in control group,however,the serum levels of CC16,sE-SLT and SAA in research group were significantly lower than those in control group(P<0.05).In addition the average values of 6WMD in research group were significantly higher than those in control group(P<0.05),and the incidence rate of adverse reaction in different time points and life quality scores in research group were significantly lower than those in control group(P<0.05).Conclusion The tiotropium bromide cpmbined with respiratory training intervention in treatment of COPD can effectively promote the recovery of lung function,improve the serum levels of CC16,SE-SLT,SAA,improve exercise endurance,reduce the incidence of adverse reactions,improve therapeutic effects and life quality of patients,thus,it is of higher clinical application value.
