肝硬化腹水证治分布及脐敷疗法用药规律文献研究

摘　　要：目的:总结肝硬化腹水的中医证型分布、治法及脐敷疗法用药规律,为中医药外治腹水提供思路。方法:检索1994—2017年关于肝硬化腹水脐敷疗法的数字化期刊,遴选符合筛选要求的文献,归纳、整理和总结其相关中医证型、治法分布及用药频次,并进行统计学处理、分析。结果:肝硬化腹水中医证型主要有:气滞湿阻、肝脾血瘀、脾肾阳虚、湿热蕴结、寒湿困脾等;中医药脐敷治疗肝硬化腹水的治法主要有:泄水逐饮,兼行气、活血、温阳、化湿等。用药频率最高为甘遂。结论:肝硬化腹水证型分布以本虚标实为主,治法以祛标为主,兼治本虚,脐敷疗法用药明显核心化,以逐水、活血、理气、温阳药为主。Objective:To provide the thinking for TCM external therapy in treating ascites by summarizing the distribution of TCM syndromes and therapeutic methods of cirrhosis ascites,medication rules of umbilical compressing therapy.Methods:After searching digital periodicals about umbilical compressing therapies for cirrhosis ascites from 1994 to 2017,the literature which met the inclusion standard was screened to conclude,collect and summarize the related TCM patterns,the distribution of therapeutic methods and medication frequency,and perform statistical management and analysis.Results:The syndromes of cirrhosis ascites mainly contained:Qi stagnation and damp obstruction,blood stasis of liver and spleen,Yang deficiency of spleen and kidney,damp and heat accumulation,cold-damp encumbering spleen,etc;therapeutic approaches of TCM umbilical compressing therapies in treating cirrhosis ascites included:removing water retention by purgation,complicated with moving qi,activating blood,warming Yang,resolving dampness,etc.The drug with the highest frequency was Gansui(Euphorbia kansui).Conclusion:The distribution of the syndromes of cirrhosis ascites is deficiency in origin and excess in superficislity,therapeutic approach adopts eliminating the superficial cause mainly,complicated by treating deficiency in origin,umbilical compressing therapy uses the core drugs notably,principally discharging water,activating blood,regulating Qi and warming Yang.
