摘　　要：目的研究Jagged2配体对结核病患者Th1/Th2平衡的影响。方法收集2019年5-12月在新疆维吾尔自治区人民医院诊治的35例肺结核患者(TB组)、28例未感染者(TST-组)和30例结核菌潜伏感染者(LTBI组)外周血,应用人淋巴细胞分离液分离外周血单个核细胞(PBMCs),应用实时荧光定量PCR法检测Jagged1和Jagged2mRNA表达情况,计算出Th1和Th2细胞占CD4+T细胞比例。根据上述结果,Pearson法分析肺结核患者外周血中Jagged2与Th1或Th2的相关性;应用Jagged2蛋白上调或siRNA-Jagged2下调Jagged2表达,应用ELISA法检测培养上清液中干扰素-γ(IFN-γ)、白介素-4(IL-4)浓度变化。结果TB组Jagged2 mRNA相对表达量分别高于TST-组和LTBI组,差异有统计学意义(P<0.05),TB组Jagged1表达分别与TST-组和LTBI组比较,差异无统计学意义(P=0.067、0.200)。TB组Th2细胞占CD4+T细胞的比例分别明显高于TST-组和LTBI组,差异有统计学意义(P<0.05);TB组Th1细胞比例分别与LTBI组和TST-组比较,差异无统计学意义(P=0.434、0.145);TB患者Th2/Th1比例明显高于TST-组和LTBI组,差异有统计学意义(P<0.05)。Jagged2、Th2、Th1和Th2/Th1在TST-和LTBI组之间比较,差异均无统计学意义(P>0.05)。Pearson分析显示:Jagged2与Th2、Th2/Th1比例呈正相关,而与Th1不相关。进一步上调或下调结核病患者PBMCs中Jagged2表达,发现培养上清液中IL-4浓度发生明显变化,IFN-γ变化不明显。结论Jagged2可能参与肺结核患者外周血中Th1/Th2失衡,尤其与Th2细胞比例异常升高相关。Objective To study the effect of Jagged2 ligand on Th1/Th2 balance in tuberculosis patients.Methods Peripheral blood of 35 patients with pulmonary tuberculosis(TB group),28 individuals with TST-and 30 individuals with LTBI group were collected from May to December 2019 in Peoples.s Hospital of Xinjiang Uygur Autonomous Region.Peripheral blood mononuclear cells(PBMCs)were isolated with human lymphocyte separation solution.Expressions of Jagged1 and 2 mRNA were detected by real-time quantitative PCR.According to the above results,Pearson method was used to analyze the correlation between Jagged2 and Th1 or Th2 in patients with tuberculosis.siRNA-Jagged2 was used to downregulate the expression of Jagged2.The changes of interferon-γ(IFN-γ)and interleukin-4(IL-4)concentrations in culture supernatant were detected by ELISA.Results The relative expression level of Jagged2 mRNA in TB group was significantly higher than those in TST-group and LTBI group(P<0.05).The expression level of Jagged1 in TB group was not statistically significant different form those in TST-group and LTBI group(P=0.067,0.200).The proportion of Th2 cells in CD4+T cells in TB group was significantly higher than those in TST-group and LTBI group(P<0.05).The proportion of Th1 cells in TB group was not statistically significant different form those in LTBI group and TST-group(P=0.434,0.145).The proportion of Th2/Th1 in TB patients was significantly higher than thsoe in TST-group and LTBI group(P<0.05).Jagged2,Th2,Th1 and Th2/Th1 of the TST-and LTBI groups were not significantly different(P>0.05).Pearson analysis shows that Jagged2 was positively correlated with the ratio of Th2 and Th2/Th1,but not with Th1.When the expression of Jagged2 in PBMCs of TB patients was further up-regulated or down-regulated,it was found that the concentration of IL-4 changed significantly,not that of IFN-γ.Conclusion Jagged2 might be involved in Th1/Th2 imbalance in pulmonary tuberculosis patients,especially related to the abnormal increase of Th2 cell ratio.
