二甲双胍联合维生素D对PCOS患者胰岛素抵抗及子宫螺旋动脉血流动力学指标的影响

摘　　要：目的:分析二甲双胍联合维生素D对多囊性卵巢综合征(PCOS)患者胰岛素抵抗及子宫螺旋动脉血流动力学指标的影响。方法:选取102例PCOS患者,按照随机数字表法分为两组,每组各51例。对照组给予二甲双胍治疗,观察组在此基础上联合使用维生素D治疗。比较治疗前后两组患者激素水平、胰岛素抵抗指标及子宫螺旋动脉血流动力学指标变化情况。结果:治疗3个月后,两组患者LH、PRI、T、FINS、HOMA-IR、PI、RI水平均低于治疗前,且观察组低于对照组(P<0.05);两组患者FSH、FPG水平比较差异无统计学意义(P>0.05);两组患者E2水平较治疗前升高,且观察组高于对照组(P<0.05)。结论:二甲双胍联合维生素D治疗PCOS效果显著,可有效改善患者性激素水平和胰岛素抵抗,恢复子宫螺旋动脉血流动力学。Objective:To analyze the effects of metformin combined with vitamin D on insulin resistance and hemodynamic indexes of uterine spiral artery in patients with polycystic ovary syndrome(PCOS).Methods:Totally,102 patients with PCOS were randomized into the observation group and control group,with 51 cases in each group.Patients in control group were treated with metformin,while patients in the observation group were additively treated with vitamin D on this basis.The levels of hormones,insulin resistance indexes and hemodynamic indexes of uterine spiral artery were compared between the two groups before and after treatment.Results:After 3 months of treatment,the levels of LH,PRI,T,FINS,HOMA-IR,PI and RI in the two groups were lower than those before treatment,and the levels of observation group were lower than those of control group(P<0.05);there were no statistically differences in the levels of FSH and FPG in patients between the two groups(P>0.05);E2 level of the two groups was increased compared with that before treatment,and the level of observation group was higher than that of control group(P<0.05).Conclusion:Metformin combined with vitamin D has significant effects in the treatment of patients with PCOS,and it can effectively improve the levels of sex hormones and insulin resistance,and restore the blood flow hemodynamics of uterine spiral artery.
