CT引导下经皮细针穿刺活检在肺部空洞性肿块患者中的敏感度、特异度和准确度研究

摘　　要：目的:探讨CT引导下经皮细针穿刺活检在肺部空洞性肿块患者中的敏感度、特异度和准确度。方法:选择某院接受治疗的肺部空洞性肿块患者64例,均对其实施CT引导下经皮细针穿刺活检,将穿刺活检结果与病理结果比较分析,评价其诊断肺部空洞性肿块穿刺的敏感度、特异度及准确度,并根据患者病灶大小(A1组17例,A2组47例)、空洞壁厚度(B1组20例,B2组44例)及穿刺针长短(C1组36例,C2组28例)分析不同组别诊断的敏感度、特异度及准确度。结果:64例肺部空洞性肿块患者经手术病理学组织检查结果确诊60例,包括恶性病变50例、良性病变10例。CT引导下经皮细针穿刺活检诊断的敏感度为96.15%、特异度为83.33%、准确度为93.73%。其敏感度、特异度、准确度与不同病灶大小、不同空洞壁厚度、不同穿刺针长度无关(P>0.05)。结论:肺部空洞性肿块患者CT引导下经皮细针穿刺活检检查效果理想,诊断率较高,值得推广应用。Objective:To observe and analyze the sensitivity,specificity and accuracy of CT guided percutaneous fine needle aspiration biopsy in patients with pulmonary cavity mass.Methods:64 patients with pulmonary cavitary mass were selected and underwent CT guided percutaneous fine needle aspiration biopsy.The results of the biopsy and pathology were analyzed,and the sensitivity,specificity and accuracy of the aspiration biopsy were assessed.The differences of sensitivity,specificity and accuracy between different groups were compared according to the size of lesions,the thickness of cavity wall and the length of puncture needle.Results:60 of the 64 patients with pulmonary cavitary mass were confirmed by surgical pathology,including 50 cases of malignant lesions and 10 cases of benign lesions.The diagnostic sensitivity,specificity and accuracy of CT guided percutaneous fine needle aspiration biopsy were 96.15%,83.33%and 93.73%respectively.There were no significant differences between the sensitivity,specificity and accuracy and the size of lesions,the thickness of cavity wall and the length of puncture needle(P>0.05).Conclusion:The result of diagnosis of pulmonary cavity mass by CT guided percutaneous fine needle aspiration biopsy is ideal and has high efficiency.It is worthy of popularization and clinical application.
