三期梅毒致舌鳞状细胞癌一例

摘　　要：梅毒可能在原发性和继发性出现多种口腔表现,并可能与恶性肿瘤混淆。尽管梅毒的发病率有所上升,但三期梅毒却极为罕见。第三期梅毒口腔受累通常在硬腭或者牙龈,而舌头受累是非常罕见的。本文介绍一名55岁男性,有常年吸烟和饮酒史,舌溃疡肿块并造成舌裂,进行正电子发射断层扫描提示恶性肿瘤,组织学显示肉芽肿性炎症。进一步的实验室结果表明,梅毒螺旋体免疫球蛋白G抗体的快速血浆Reagin滴度升高。该患者被诊断为三期梅毒,接受了适当的抗生素治疗,并舌裂有持续的愈合。Syphilis may present with multiple oral manifestations in primary and secondary schools,which may be confused with malignant tumors.Although the incidence of syphilis has increased,tertiary syphilis is extremely rare.Oral involvement of tertiary syphilis is usually in the hard palate or gums,while tongue involvement is very rare.We present the case of a 55-year-old man with a history of chronic tobacco and alcohol use,a tongue ulcer mass with a cracked tongue,a malignant tumor on positron emission tomography and granulomatous inflammation on histology.Further laboratory results showed an increase in rapid plasma Reagin titers of Treponema pallidum immunoglobulin G antibody.The patient was diagnosed with tertiary syphilis,treated with appropriate antibiotics,and had sustained healing of the split tongue.
