摘 要:目的分析2016年和2019年北京市昌平区结核病防治所就诊患者的耐药情况.方法对2016年和2019年痰培养阳性患者分离株进行固体药物敏感性试验,并对其相应药敏结果的一线及二线抗结核药物情况、非结核分枝杆菌构成比及非结核风险因素等指标进行系统分析.结果北京昌平地区2016年和2019年不同药物耐药率之间无统计学意义(P>0.05).耐多药结核病(MDR-TB)的比率分别是2.07%(5/243)和3.68%(7/190),两年份之间耐多药比率无统计学意义(P>0.05).2016年耐药比例为14.88%(36/242),敏感菌株比例为85.12%(206/242),2019耐药比例为32%(54/190),敏感菌株比例为68%(136/190),两年份之间总的耐药比例具有统计学差异(P<0.05).2016年和2019年非结核分离株比例分别为1.22%(3/245)和5.00%(10/200).其中在非结核病风险因素分析中,男性相比于女性不易感染非结核分枝杆菌(OR=0.6795),年龄<45岁的人群相比年龄≥45的人群不易感染非结核分枝杆菌(OR=0.2289).结论耐药结核病在本地区仍然是重要挑战,且本地区非结核病呈现一种上升趋势,此地区女性和45岁及以上人群仍是非结核病监测和防控关注的重点对象.Objective Analysis of drug resistance in patients attending the Beijing Changping Institute for Tuberculosis Prevention and Treatment in 2016 and 2019.Met hods Solid drug susceptibility tests were performed on sputum culture-positive patient isolates in 2016 and 2019,and indicators such as first-and second-line antituberculosis drug profiles,non-tuberculosis composition ratio and non-tuherculosis risk factors were analyzed systematically for their corresponding drug sensitivity results.Results There is no statistical significance(P>0.05)between various drug resistance rates in 2016 and 2019 in Beijing Changping area.The rates of multidrug-resistant TB(MDR-TB)is 2.07%(5/243)and 3.68%(7/190),respectively,with no statistically significant rate of multiclrug resistance between the two years(P>0.05).The percentage of resistance in 2016 is 14.88%(36/242)and the percentage of sensitive strains is 85.12%(206/242),the percentage of resistance in 2019 is 32%(54/190)and the percentage of sensitive strains is 68%(136/190),The total percentage of resistance is statistically different between the two years(P<0.05).The proportion of non-tuberculous isolates is 1.22%(3/245)and 5.00%(10/200)in 2016 and 2019.respectively.In the analysis of nontuberculosis risk factors,men are less likely to be infected with non-tuberculous mycobacteria compared to women(OR=0.6795),and those aged<45 years are less likely to be infected with non-tuberculous mycobacteria compared to those aged N45 years(OR=0.22X9).Conclusion Drug-resistant TB remains an important challenge in the region,and there is an upward trend in non-TB disease in the region,women and the aged 45 years and older in this region remain a priority for non-TB surveillance and prevention and control concerns.
