腔内置管与全身静脉溶栓治疗下肢深静脉血栓形成的安全性分析

摘　　要：目的探究对下肢深静脉血栓形成患者实施腔内置管溶栓、全身静脉溶栓治疗的效果。方法使用随机数字表法将2017年6月至2019年4月新疆生产建设兵团第一师医院68例下肢深静脉血栓形成患者分为对照组(34例,应用全身静脉溶栓治疗)、实验组(34例,应用腔内置管溶栓治疗)。随访半年,对比两组溶栓总有效率、肢体周径差、静脉通畅评分、静脉完全通畅率、消肿率、PTS(血栓形成后综合征)发生率及不良事件发生率。结果实验组溶栓总有效率(94.12%)高于对照组(70.59%),P <0.05;实验组治疗后7d大腿周径差、小腿周径差均较对照组更小,P <0.05;实验组治疗后7d静脉通畅评分较对照组更低,且实验组静脉完全通畅率、6个月后消肿率高于对照组,6个月后PTS发生率较对照组更低,P <0.05;实验组不良事件发生率(2.94%)低于对照组(23.53%),P <0.05。结论对下肢深静脉血栓形成患者实施腔内置管溶栓治疗的效果较全身静脉溶栓更佳,安全性更高。Objective To explore the effect of intravascular thrombolysis and systemic thrombolysis in patients with lower extremity deep vein thrombosis. Methods 68 patients with deep vein thrombosis of the lower extremities in our hospital from June 2017 to April2019 were divided into the control group(34 cases)and the experimental group(34 cases)according to random number table method.Control group were treated with systemic intravenous thrombolysis therapy, while experimental group were applied intracavitary tube thrombolytic therapy. The total effective rate of thrombolysis, limb circumference difference, venous patency score, complete venous patency rate, swelling rate, PTS(post-thrombosis syndrome)rate, and adverse event rate were compared between the two groups after following up for half a year,. Results The total effective rate of thrombolysis in the experimental group(94.12%)was higher than that of the control group(70.59%),(P<0.05). The difference in thigh circumference and calf circumference in the experimental group were smaller than those in the control group at 7 days after treatment,(P<0.05). The venous patency score of the experimental group was lower than that of the control group at 7 days after treatment, and the rate of complete venous patency and swelling rate in the experimental group were higher than those of the control group after 6 months. The incidence of PTS was lower than that of the control group after 6 months,(P< 0.05). The incidence of adverse events in the experimental group(2.94%)was lower than that in the control group(23.53%),(P<0.05). Conclusion The effect of intravascular thrombolysis is better than that of systemic thrombolysis in patients with DVT.
