摘 要:目的探讨B超多参数与宫高腹围法预测胎儿体重的价值。方法选择2020年1月至2020年6月在本院行常规孕检的190例孕妇,分娩前7d内行B超与宫高腹围法预测胎儿体重。以胎儿出生体质量为金标准,比较B超多参数与宫高腹围法胎儿体重、巨大儿符合率,分析B超多参数与宫高腹围法预测胎儿体重效能。结果 B超多参数法预测胎儿体重符合率为90.00%,高于宫高腹围法的71.05%,差异有统计学意义(P <0.05);190例新生儿中共分娩巨大儿38例(20.00%),B超多参数法预测巨大儿符合率为89.47%,高于宫高腹围法的68.42%,差异有统计学意义(P <0.05);宫高腹围法预测胎儿体重曲线下面积(AUC)为0.720(95%CI:0.641-0.8.05),敏感度和特异度分别为63.47%、76.20%;B超多参数法预测胎儿体重AUC为0.836(95%CI:0.679-0.993),敏感度和特异度分别为92.72%、81.45%。结论 B超多参数预测胎儿体重准确度、敏感度和特异度较高,能有效检出巨大儿,为分娩提供参考依据。Objective To explore the value of B-ultrasound multi-parameter and uterine height/abdominal circumference method in predicting fetal weight. Methods Ninety pregnant women who underwent routine pregnancy check in our hospital from February 2019 to March 2020 were selected. B-ultrasound and uterine height/abdominal circumference method were performed to predict fetal weight within7 days before delivery. Taking the birth weight of the fetus as the gold standard, the coincidence rate of B-ultrasound multi-parameters with the uterine height and abdominal circumference method for fetal weight and macrosomia were compared, and the efficacy of B-ultrasound multi-parameters and the uterine height and abdominal circumference method for predicting fetal weight were analyzed.Results The coincidence rate of fetal weight predicted by the B-ultrasonic multi-parameter method was 90.00%, which was higher than the 71.05% of the uterine height and abdominal circumference method, and the difference was statistically significant(P < 0.05). Among190 newborns, 38 cases were belonging to giant babies(20.00%). The coincidence rate of macrosomia predicted by B-ultrasound multi-parameter method were 89.47%, which was higher than 68.42% through uterine height/abdominal circumference method, and the difference was statistically significant(P < 0.05). The uterine height/abdominal circumference method predicted the area under the fetal weight curve(AUC)is 0.720(95%CI: 0.641-0.8.05), the sensitivity and specificity are 63.47% and 76.20% respectively. B-ultrasound multi-parameter method predicted fetal weight AUC of 0.836(95%CI: 0.679-0.993), the sensitivity and specificity were 92.72% and81.45%, respectively. Conclusion B-ultrasound multi-parameter prediction of fetal weight has high accuracy, sensitivity and specificity,which can effectively detect macrosomia and provide a reference for delivery.
