2DSTI技术在评估H型高血压颈动脉粥样硬化患者血管壁弹性变化及功能的应用

摘  要:目的利用超声二维斑点追踪(2DSTI)技术评估H型高血压患者颈动脉弹性变化,发现颈动脉管壁硬化程度与弹性参数之间的相关性。方法将2019年5月至2020年9月间在佳木斯市中心医院神经内科住院和门诊治疗的H型高血压患者60例[分为观察一组(H型高血压伴颈动脉粥样硬化)和观察二组(H型高血压无动脉粥样硬化)],另选30例单纯性原发性高血压(EH)患者作为对照组。三组均行高频超声和2DSTI技术检查,在血管短轴切面测量并记录颈动脉血管壁运动度(ΔD)、阻力指数(RI)及内-中膜厚度(IMT)、僵硬度参数(β)、整体及各个节段(11~1点、5~7点、9~11点)的圆周应变(CS)、圆周应变率(CSR)并获得CS、CSR-时间的ROC曲线。结果与照组相比,观察一组和二组的CS、CSR及11~1点、5~7点、9~11点CS、CSR明显降低(P<0.01),ΔD减低(P<0.05),β与IMT均增加(P<0.05),RI增加(P>0.05);观察组间比较整体CS、CSR以及11~1点、5~7点、9~11点CS、CSR降低(P<0.05),β及IMT增加(P<0.05),ΔD、RI无显著差异(P>0.05);观察二组与对照组比较,IMT增加,无统计学意义(P>0.05)。H型高血压颈动脉粥样硬化血管的弹性变化与整体CS、CSR呈负相关、与β呈正相关(P<0.05)。结论2DSTI技术能较好评价H型高血压患者颈动脉管壁的弹性改变和管壁功能改变,为H型高血压颈动脉血管病变的早期诊断和干预治疗提供了新的依据。Objective To evaluate the correlation the between the degree of carotid atherosclerosis and elasticity paremeters of carotid artery in patients with H-type hypertension by Two-dimensional speckle-tracking(2DSTI).Methods From May 2019 to September 2020,60 patients with H-type hypertension were treated in Neurology Inpatients and outpatients of Jiamusi Central Hospital.They were divided into observation group 1(had carotid atherosclerosis)and observation group 2(without carotid atherosclerosis),while 30 patients with essential hypertension(EH)selects as the control group.2DSTI and high frequency ultrasound were used to examine all three groups.The carotid of overall peak CS,CSR and various segments(11~1point,5~7point,9~11point),IMT,RI,β,ΔD were measured and recorded in the short-axis section of the vessels and ROC curves got the CS,CSR-time.Results Compared with the control group,the overall CS,CSR and 11~1 points,5~7 points,9~11 points of CS,CSR in the observation group 1 and group 2 were significantly decreased(P<0.01),ΔD decreased(P<0.05),βAnd IMT increased(P<0.05),RI increased(P>0.05).The overall CS,CSR and CS,CSR at 11~1 points,5~7 points and 9~11 points were decreased in the observation group(P<0.05),βAnd IMT increased(P<0.05),βAnd IMT increased(P<0.05),the difference of RI、ΔD was no statistically significant(P>0.05).Compared with the control group,IMT of the observation group 2 increased,the difference was statistically significant(P>0.05).The elasticity of carotid artery in H-type hypertension was negatively correlated with CS,CSR,βpositive correlation(P<0.05).There was no significant statistic difference in the increase of IMT between the group 2 and the control group(P>0.05).The elasticity of carotid artery in H-type hypertension was negatively correlated with overall CS,CSR and positively correlation withβ(P<0.05).Conclusion Two-dimensional speckle-tracking can better evaluator of the carotid artery wall elasticity and function in patients with H-type hypertension;it will provide a new basis f
