盐酸舍曲林改善稳定期慢性心力衰竭患者心功能的对照研究

摘　　要：目的探讨盐酸舍曲林对慢性心力衰竭(Chronic heart failure,CHF)患者心功能的改善作用。方法选取2019年3月至2020年3月间,在江苏省荣军医院住院治疗,以及在梁溪区接受家庭康复的稳定期CHF同时存在焦虑和抑郁症状的患者64例,采用随机化方法,分成对照组和药物组,每组32例。对照组患者予以常规抗心衰治疗,药物组患者在对照组治疗的基础上联合盐酸舍曲林治疗。通过汉密顿焦虑量表(Hamilton anxiety scale,HAMA)和汉密顿抑郁量表(Hamilton depression scale,HAMD)分别评估两组患者治疗6周前后的焦虑、抑郁得分及心功能指标[左心室射血分数(Left ventricular ejection fraction,LVEF)、6分钟步行实验(6-minute walk test,6MWT)、最大摄氧量(VO2max)、N末端B型利钠肽(Btype natriuretic peptide,BNP)(NT-proBNP)]。结果治疗前,两组患者在焦虑、抑郁及心功能方面的差异均无统计学意义(P>0.05)。治疗6周后,药物组患者HAMA总分明显降低,且显著低于对照组,差异均有统计学意义(均P<0.05);药物组患者在LVEF、6MWT、VO2max、NT-proBNP方面的改善明显优于对照组,差异存在统计学意义(P<0.05)。结论盐酸舍曲林在短期内未能积极改善CHF患者的不良情绪,但可明显提高CHF患者的心功能水平。Objective To investigate the effect of sertraline hydrochloride on cardiac function in patients with chronic heart failure(CHF).Methods From March 2019 to March 2020,64 stable CHF patients with anxiety and depression were randomly divided into control group and drug group,32 cases in each group.The control group was given conventional anti heart failure treatment,and the drug group was treated with sertraline hydrochloride based on the control group.Hamilton Anxiety Scale(HAMA)and Hamilton Depression Scale(HAMD)were used to evaluating the scores of the two groups before and after sertraline hydrochloride treatment for 6 weeks.The cardiac function indexes including left ventricular ejection fraction(LVEF),6-minute walk test(6MWT),VO 2max and N-terminal B-type natriuretic peptide(NT-proBNP)were compared between the two groups before and after sertraline hydrochloride treatment for 6 weeks.Results Before treatment,there were no significant differences in anxiety,depression and cardiac function between the two groups(P>0.05).After treatment,the HAMA total score of the drug group was significantly lower than that of the control group(P<0.05);the improvement of LVEF、6MWT、VO 2max and NT-proBNP in the drug group was significantly better than that in the control group(P<0.05).Conclusion Sertraline hydrochloride can improve heart function in patients with CHF.Conclusion Sertraline hydrochloride can improve heart function in patients with CHF.
