摘　　要：目的分析山东省某三甲医院医护人员防御性医疗行为现状及其影响因素,为管理者规范防御性医疗行为提供决策依据。方法通过对此三甲医院的医护人员进行问卷调查收集资料,并应用描述性分析,单因素分析和多元回归模型进行统计分析。结果本次调查对象422人中,99.29%的医护人员在日常工作中采取防御性医疗行为。职业和近5年医疗纠纷经历对积极防御性医疗行为得分的影响具有统计学意义(P<0.05),医生的得分显著低于护士,有纠纷经历的医护人员得分高于无经历者的得分;工作年限和科室对消极防御性医疗行为得分的影响具有统计学意义(P<0.05),工作年限越长,得分越低,外科医护人员的得分显著高于其他科室;科室对总体防御性医疗行为得分的影响具有统计学意义(P<0.05),外科医护人员的得分显著高于其他科室。结论积极防御性医疗行为在医护人员中较为普遍,应通过政府、社会、医疗机构和医护人员多方共同努力,合理规范防御性医疗行为,从而提高医院的综合服务效率,为患者提供更优质的医疗服务。Objective To analyze the current situation and influencing factors of defensive medical behaviors of medical staff in a tertiary hospital of Shandong Province,and to provide decision-making basis for managers to regulate defensive medical behaviors.Methods A questionnaire survey was conducted on the medical staff in this tertiary hospital to collect data,and the descriptive analysis,univariate analysis and multiple linear regressions were used for statistical analysis.Results Out of 422 medical staff,99.29%of them adopted defensive medical behaviors at work.There was statistical significance difference in the scores of positive defensive medical behaviors of medical staff with different jobs and experience of medical disputes in recent five years(P<0.05),the score of doctors was significantly lower than that of nurses and the score of medical staff who had experienced medical disputes in recent five years was significantly higher than that of medical staff without this experience.There was statistical significance difference in the scores of negative defensive medical behaviors of medical staff with different working years and departments(P<0.05),the scores increased with the working years,and the score of medical staff in surgery departments was significantly higher than that of medical staff in other departments.There was statistical significance difference in the scores of overall defensive medical behaviors of medical staff with different departments(P<0.05),the score of medical staff in surgery departments was significantly higher than that of medical staff in other departments.Conclusion Active defensive medical behaviors are common among medical staff.The government,society,medical institutions,and staff should make joint efforts to regulate defensive medical behaviors,so as to improve the hospital comprehensive service efficiency and provide more quality services to patients.
