超声引导下细针穿刺活检在甲状腺结节诊断中的应用价值探讨

摘　　要：目的:探讨超声引导下细针穿刺活检(US-F N AB)在甲状腺结节诊断中的应用价值。方法:本文将2018年3月—2020年3月我院收治的经超声检查发现以实性为主但难定性的96例甲状腺结节患者(112枚)作为研究对象,所有患者入院后均接受超声弹性成像(U E)、US-FNAB检查,对其临床资料进行回顾分析。结果:112枚甲状腺结节术后病理或随访确诊良性结节83枚,恶性结节29枚;US-FNAB诊断的准确性、特异度、敏感度分别为92.86%、100.00%、97.50%,均显著高于UE的46.43%、28.77%、79.49%(P<0.05),误诊率、漏诊率分别为0.00%、2.50%,均显著低于UE的71.23%、20.51%(P<0.05)。结论:US-FNAB对于甲状腺结节良恶性鉴别诊断具有一定应用价值,可作为临床甲状腺结节良恶性鉴别诊断的重要依据。Objective To explore the application value of ultrasound-guided fine needle puncture biopsy(US-FNAB)in the diagnosis of thyroid nodule.Methods In this study,96 cases(112 cases)of thyroid nodule patients admitted to our hospital from March 2018 to March 2020 were found to be mostly solid but difficult to be determined by ultrasound examination.All patients received ultrasound elastography(UE)and US-FNAB examination after admission,and their clinical data were retrospectively analyzed.Results 112 thyroid nodules were confirmed by postoperative pathology or follow-up with 83 benign nodules and 29 malignant nodules.The diagnostic accuracy,specificity and sensitivity of US-FNAB were 92.86%,100.00%and 97.5%respectively,which were significantly higher than those of UE(46.43%,28.77%and 79.49%)(P<0.05).The misdiagnosis rate and missed diagnosis rate of US-FNAB were 0.00%and 2.50%respectively,which were significantly lower than those of UE(71.23%and 20.51%)(P<0.05).Conclusion US-FNAB has certain application value in differential diagnosis of benign and malignant thyroid nodules,and can be used as an important basis for clinical differential diagnosis of benign and malignant thyroid nodules.
