摘  要:目的:探讨幕上Ⅱ~Ⅳ级胶质瘤多模态MRI的分级诊断与病理学结果的相关性。方法:回顾性分析2017年1月-2019年12月的胶质瘤患者126例,其中低级别胶质瘤(Ⅱ级)56例、高级别胶质瘤(Ⅲ级和Ⅳ级)70例。所有患者均接受T1WI、T2WI、扩散加权成像(DWI)、扩散张量成像(DTI)、动态增强成像(DCE-MRI)以及动脉自旋标记灌注成像(3D-ASL)。结果:常规MRI、DWI、DTI、DCE-MRI、3D-ASL鉴别诊断高、低级别胶质瘤的敏感度分别为78.57%、91.07%、91.07%、92.86%、83.93%,特异度分别为81.43%、90.00%、91.43%、95.71%、88.57%,上述序列联合诊断的敏感度为98.21%,特异度为97.14%,与病理结果高度一致(Kappa=0.952)。结论:多模态MRI对幕上Ⅱ~Ⅳ级胶质瘤的分级诊断与病理结果高度一致。Objective:To investigate the relationship between multimodal magnetic resonance imaging(MRI)and pathology on diagnosis of gradeⅡ~Ⅳsupratentorial glioma.Methods:One hundred and twenty-six patients with glioma from January 2017 to December 2019 were enrolled and retrospectively analyzed,which included low-grade gliomas(gradeⅡ)in 56 patients and high-grade gliomas(gradeⅢandⅣ)in 70 patients.All patients performed T1-weighted imaging(T1WI),T2-weighted imaging(T2WI),diffusion-weighted imaging(DWI),diffusion tensor imaging(DTI),dynamic contrast enhanced imaging(DCE-MRI),and three dimensional arterial spin labeling(3 D-ASL).Results:The sensitivities of conventional MRI,DWI,DTI,DCE-MRI,and 3 D-ASL in the differential diagnosis of high-grade and low-grade gliomas were 78.57%,91.07%,91.07%,92.86%,and 83.93%,respectively.The specificities were 81.43%,90.00%,91.43%,95.71%,and 88.57%,respectively.The sensitivity and specificity of its combined diagnosis were 98.21%and 97.14%respectively,which had high consistency with pathologic results(Kappa=0.952).Conclusion:There are high consistent in diagnosis of gradeⅡ~Ⅳsupratentorial glioma between multimodal MRI and pathology.
