脑卒中手术患者创伤后成长水平及影响因素的纵向研究

摘　　要：目的调查脑卒中手术患者不同时期的创伤后成长变化情况及影响因素,为改善其负性心理提供参考。方法选取2017年10月至2020年1月神经外科339例脑卒中手术患者作为研究对象,分别在脑卒中手术患者入院(T1)、术后6个月(T2)、术后1年(T3)3个时间点进行调查,采用一般情况调查表、创伤后成长评定量表、社会支持量表、家庭支持量表进行评测。结果T1~T3期患者创伤后成长评分依次为(34.366±5.757)分、(49.410±5.757)分、(60.520±5.751)分。多元线性回归分析结果显示,T1~T3期脑卒中手术患者创伤后成长均与年龄、性别、性格、文化程度、婚姻状况、社会支持、家庭支持有关,差异有统计学意义(P<0.05);收支状况仅在T3期对脑卒中手术患者创伤后成长产生影响,差异有统计学意义(P<0.05)。结论T1~T3期脑卒中手术患者创伤后成长水平呈上升趋势,应关注脑卒中手术患者创伤后成长变化趋势及其影响因素,预防不良情绪的产生,为针对性干预提供理论基础。Objective To investigate the changes of post-traumatic growth in patients with cerebral apoplexy at different periods and its influencing factors,so as to provide reference for improving the negative emotion of patients.Methods A total of 339 patients with cerebral apoplexy who were treated in our hospital from October 2017 to January 2020 were nerolled in the study.The investigation performed on admission(T1),at 6m after operation(T2),and at 1yr after operation(T3).And the general situation questionnaire,post-traumatic growth assessment scale,social support scale and family support scale were used to evaluate the influencing factors.Results The post-traumatic growth scores of patients at T1,T2,T3 were(34.366±5.757)points,(49.41±5.757)points,(60.520±5.751)points,respectively.The multiple linear regression analysis showed that the post-traumatic growth of patients in T1,T2,T3 was closely correlated with patient’s age and sex,personality,education background,marital status,social support,family support(P<0.05).The income and expenditure status significantly affected the post-traumatic growth of patients only at T3 stage(P<0.05).Conclusion The post-traumatic growth levels in patients with cerebral apoplexy at T1,T2,T3 show a increasing trend,it is necessary to pay much attention to the trend and its influencing factors,so as to prevent the production of unhealthy emotions,which provides a theoretical basis for targeted intervention.
