摘　　要：目的:探讨留置输尿管导管的部分无管化经皮肾镜碎石取石术(PCNL)的疗效及安全性。方法:回顾性分析2017年10月至2019年5月广州医科大学附属第二医院泌尿外科收治的50例接受部分无管化PCNL和45例接受传统PCNL患者的病历资料,比较两组术中及术后相关指标的差异。结果:两组术后血红蛋白下降值、发热例数和肾周积液例数比较差异无统计学意义(P>0.05);部分无管化组住院时间、术后疼痛评分明显低于传统组(均P<0.01)。结论:留置输尿管导管的部分无管化PCNL和传统PCNL疗效相当,但部分无管化PCNL能缩短患者住院时间,降低患者术后疼痛,且不会增加手术并发症,无须再次膀胱镜拔管。Objective:To investigate the efficacy and safety of partial tubeless percutaneous nephrolithotomy(PCNL)with indwelling ureteral catheter.Methods:The clinical data of 50 patients who underwent partial tubeless PCNL and 45 patients who underwent traditional PCNLin Department of Urology,Second Affiliated Hospital of Guangzhou Medical University were retrospectively analyzed from October 2017 to May 2019,and the differences in intraoperative and postoperative related indexes between the two groups were compared.Results:There were no significant differences between the two groups in postoperative hemoglobin decrease,cases of fever and cases of perirenal effusion(P>0.05);the hospital stay and postoperative pain score in partial tubeless groups were significantly lower than those in the traditional group(allP<0.01).Conclusion:Partial tubeless PCNL with indwelling ureteral catheter is as effective as traditional PCNL,but partial tubeless PCNL can shorten hospital stay,reduce postoperative pain without increasing surgical complications,and does not require cystoscope extubation again.
