摘　　要：目的探讨欧洲五维度健康量表(EQ-5D-5L)对维持性血液透析(MHD)患者的生活质量与心理社会因素的评估价值。方法选用EQ-5D-5L对2018年7月~10月在四川大学华西医院肾内科血液透析中心接受维持性血液透析的176例患者进行问卷调查,分析其与一般资料、匹兹堡睡眠质量指数量表、综合医院焦虑抑郁量表(HADS)、社会支持评定量表、Piper疲乏修订量表的相关性,并进行单因素和多因素分析其与影响因素的相关性。结果MHD患者EQ-5D-5L得分为(0.86±0.12)分。匹兹堡睡眠质量指数得分、HADS得分、Piper疲乏量表得分与ED-5Q-5L得分均呈负相关(P<0.05),社会支持得分与ED-5Q-5L得分呈正相关(P<0.05)。患者HADS、年龄、疲乏、透析后有无工作均是ED-5Q-5L的影响因子(P<0.05)。结论MHD患者HADS、年龄、疲乏、失业均影响其生活质量,焦虑抑郁、睡眠质量及疲乏程度越严重,生活质量就越低,患者得到的社会支持越多,生活质量越高。EQ-5D-5L对MHD患者健康相关生活质量与心理社会因素的评估具有重要价值,可在临床推广应用,以提高MHD患者的生活质量。Objective To investigate the value of EQ-5D-5L in the evaluation of life quality and psychosocial factors of maintenance hemodialysis(MHD)patients.Methods A questionnaire survey was conducted on 176 patients who received maintenance hemodialysis in the hemodialysis center of West China Hospital of Sichuan University by using European Five-dimensional Health Scale(EQ-5D-5L).The association with general data,Pittsburgh Sleep Quality Index,General Hospital Anxiety and Depression Scale(HADS),Social Support Rating Scale,and Piper Fatigue Rating Scale was analyzed.Univariate and multivariate analyses were carried out on the correlation between the factors and the influencing factors.Results The EQ-5D-5L score of MHD patients was(0.86±0.12).Pittsburgh sleep quality Index score,HADS score and Piper fatigue scale score were negatively correlated with EQ-5D-5L(P<0.05),while social support score was positively correlated with EQ-5D-5L(P<0.05).Patients with HADS,age,fatigue,and work after dialysis were all the influencing factors of EQ-5D-5L(P<0.05).Conclusion HADS,age,fatigue and unemployment all affect the quality of life of MHD patients.The more serious anxiety,depression,sleep quality and fatigue they had,they had poor the quality of life.If they got the more social support,they would have led the higher quality of life.EQ-5D-5L is of great value in the assessment of the quality of life and psychosocial factors of MHD patients,and can be popularized and applied in clinical practice to improve the quality of life of MHD patients.
