摘 要:目的系统评价双歧杆菌预防早产儿坏死性结肠炎(NEC)的有效性与安全性。方法计算机检索PubMed、Embase、Cochrane Central Register of Controlled Trials、Medline、CBM和CNKI;全面收集双歧杆菌预防早产儿NEC的安全性与有效性研究,用Reviewmanager 5.0软件进行meta分析。结果纳入22项随机对照临床研究,共计5847例患者;meta分析结果表明,双歧杆菌可有效降低早产儿NEC发生率(RR=0.42,95% CI:0.28~0.63,P<0.0001)和总病死率(RR=0.74,95% CI:0.60~0.92,P=0.006),双歧杆菌组和对照组的脓毒症发生率差异无统计学意义(RR=0.87,95% CI:0.73~1.03,P=0.11)。结论双歧杆菌可有效、安全预防早产儿NEC发生。Objective To systematically evaluate the efficacy and safety of bifidobacteria in the prevention of necrotizing enterocolitis(NEC)in premature infants based on evidence-based pharmacy.Methods PubMed,Embase,Cochrane Central Register of Controlled Trials,Medline,CBM,and CNKI were searched by the computer.A comprehensive collection of studies on the efficacy and safety of bifidobacteria in the prevention of NEC in premature infants was conducted,and the meta-analysis was performed by Review Manager 5.0 software.Results Twenty-two randomized controlled clinical trials involving 5847 patients were included.Meta-analysis results showed that bifidobacteria could effectively reduce the incidence of NEC in premature infants(RR=0.42,95% CI:0.28-0.63,P<0.0001)and the total mortality(RR=0.74,95% CI:0.60-0.92,P=0.006);there was no statistically significant difference in the incidence of sepsis between the bifidobacteria group and the control group(RR=0.87,95% CI:0.73-1.03,P=0.11).Conclusion Bifidobacterium can effectively and safely prevent the occurrence of NEC in premature infants.
