加味桃核承气汤治疗脓毒症胃肠功能障碍的临床研究

摘　　要：目的评价加味桃核承气汤治疗脓毒症胃肠功能障碍的临床疗效及安全性。方法将60例患者随机分为治疗组与对照组各30例,两组均予以西医基础治疗,治疗组在其基础上加用加味桃核承气汤中药配方颗粒。治疗7 d后,观察两组胃肠功能障碍评分、腹内压、白细胞计数(WBC)、超敏C反应蛋白(CRP)、降钙素原(PCT)、血清淀粉样蛋白A(SAA)、急性生理与慢性健康状况Ⅱ(APACHEⅡ)评分变化以及28 d生存率。结果与治疗前相比,两组胃肠功能障碍评分、腹内压、WBC、CRP、PCT、SAA、APACHEⅡ评分均明显下降(P<0.05)。两组相比,治疗组较对照组胃肠功能障碍评分、腹内压、WBC、CRP、APACHEⅡ评分改善更加明显(P<0.05);在PCT、SAA方面,两组无明显差异(P>0.05);两组28 d生存率无明显差异(P>0.05)。结论加味桃核承气汤能够促进脓毒症胃肠功能障碍患者胃肠功能恢复,改善病情,并且具有较好的临床安全性。Objective:To evaluate the clinical efficacy and safety of Modified Taohe Chengqi Decoction in the treatment of gastrointestinal dysfunction in sepsis.Methods:A total of 60 patients with sepsis gastrointestinal dysfunction who met the inclusion criteria were randomly divided into the treatment group and the control group.Both groups were given western medicine basic treatment;on this basis,the treatment group was added with Modified Taohe Chengqi Decoction.After 7 days of treatment,the changes of gastrointestinal dysfunction score,intra-abdominal pressure,WBC,CRP,PCT,SAA,APACHEⅡscore and 28 day survival rate were observed.Results:Compared with before treatment,the scores of gastrointestinal dysfunction,intra-abdominal pressure,WBC,CRP,PCT,SAA and APACHEⅡin the two groups were significantly decreased(P<0.05).Compared with the control group,the gastrointestinal dysfunction score,intra-abdominal pressure,WBC,CRP and APACHEⅡscore of the treatment group were improved more significantly(P<0.05).There was no significant difference in PCT and SAA between the two groups(P>0.05).There was no significant difference in the 28 day survival rate between the two groups(P>0.05).Conclusion:Modified Taohe Chengqi Decoction can promote the recovery of gastrointestinal function in patients with sepsis gastrointestinal dysfunction,improve the condition,and has good clinical safety.
