清肺养阴活血方联合泼尼松龙治疗急性放射性肺炎疗效观察

摘　　要：目的观察清肺养阴活血方联合泼尼松龙治疗急性放射性肺炎(ARP)的临床疗效并探讨其作用机制。方法64例ARP患者随机分为观察组与对照组各32例。对照组予以泼尼松龙治疗,观察组在对照组治疗基础上增用清肺养阴活血方。两组均以7 d为1个疗程,治疗2个疗程。比较两组临床疗效,观察两组治疗前后的主要症状评分、炎性因子水平和RTOG评分。结果观察组总有效率为93.75%,高于对照组的75.00%(P<0.05)。两组治疗前主要症状评分比较,差异无统计学意义(均P>0.05),两组治疗后主要症状评分均较治疗前下降(P<0.05),且观察组治疗后主要症状评分低于对照组(P<0.05)。两组治疗前炎性因子水平比较,差异无统计学意义(均P>0.05),两组治疗后炎性因子水平均较治疗前升高(P<0.05),且观察组治疗后炎性因子水平均低于对照组(P<0.05)。两组治疗前RTOG评分比较,差异无统计学意义(P>0.05),观察组治疗后7、14 d的RTOG评分均低于治疗前,且观察组RTOG评分均优于对照组同期(P<0.05)。结论使用清肺养阴活血方联合泼尼松龙治疗ARP患者疗效良好,可有效改善患者临床症状,减轻炎症反应和肺损害程度。Objective:To observe the clinical efficacy of Qingfei Yangyin Huoxue Decoction combined with prednisolone in the treatment of acute radiation pneumonia(ARP).Methods:A total of 64 ARP patients were divided into observation group and control group by random number table method,32 cases in each group.The control group was treated with prednisolone,and the observation group was treated with Qingfei Yangyin Huoxue Decoction on the basis of the control group.7 days was a course of treatment and the patients were treated for 2 courses.The clinical efficacy of the two groups was compared,and the differences in the main symptom scores,inflammatory factor levels and RTOG scores between the two groups before and after treatment were observed.Results:The total effective rate of the observation group(93.75%)was significantly higher than that of the control group(75.00%)(P<0.05).The main symptom scores of the observation group after treatment were significantly lower than those of the control group(P<0.05).After treatment,the level of inflammatory factors in the observation group was significantly lower than those in the control group(P<0.05).The RTOG scores of the observation group at 7 d and14 d after treatment were significantly lower than those of the control group(P<0.05).Conclusion:The treatment of ARP patients with Qingfei Yangyin Huoxue Decoction combined with prednisolone has a good effect,which can effectively improve the clinical symptoms of patients and reduce the degree of inflammation and lung damage.
