摘  要:目的研究语前聋人工耳蜗植入患儿佩戴人工耳蜗装置的康复疗效。方法选取2016年2月~2019年2月我院收治的语前聋人工耳蜗植入患儿80例作为研究对象。分别于佩戴人工耳蜗装置前以及佩戴6个月后,通过听觉能力分级(categories of auditory performance, CAP)、言语可懂度分级(speech intelligibility rating, SIR)对患儿的康复疗效进行评估。并对CAP、SIR影响因素进行单因素分析以及多因素Logistic回归分析。结果干预后患儿CAP、SIR得分均高于干预前(P均<0.05)。单因素分析发现:人工耳蜗植入年龄、家长受教育程度、家庭月收入均是语前聋人工耳蜗植入术患儿CAP得分的影响因素(P均<0.05)。经多因素Logistic回归分析可得:人工耳蜗植入年龄<5岁、家长受教育程度在高中及以下、家庭月收入<3000元均是语前聋人工耳蜗植入术患儿CAP得分的独立危险因素(P均<0.05)。单因素分析发现:人工耳蜗植入年龄、家长受教育程度、家庭月收入均是语前聋人工耳蜗植入术患儿SIR得分的影响因素(P均<0.05)。经多因素Logistic回归分析可得:人工耳蜗植入年龄<5岁、家长受教育程度在高中及以下、家庭月收入<3000元均是语前聋人工耳蜗植入术患儿SIR得分的独立危险因素(P均<0.05)。结论语前聋人工耳蜗植入患儿佩戴人工耳蜗装置的康复疗效显著,且人工耳蜗植入年龄、家长受教育程度、家庭月收入均是影响康复疗效的相关因素。Objective To study the rehabilitation effect of cochlear implant on children with preverbal deafness. Methods From February 2016 to February 2019, 80 cases of preverbal deafness children with cochlear implantation were included as subjects. The rehabilitation efficacy of the children was evaluated by categories of auditory performance(CAP) and speech intelligibility rating(SIR) before and after the cochlear implant was worn for 6 months respectively. Univariate analysis and multivariate Logistic regression analysis of the influencing factors of CAP and SIR were carried out. Results The scores of CAP and SIR were higher after intervention than before intervention(P<0.05). Single-factor analysis showed that the age of cochlear implantation, the education level of parents and the monthly income of families were all influencing factors of CAP score in children with pre-verbal deafness and cochlear implantation(all P<0.05). According to multivariate Logistic regression analysis, the age of cochlear implantation <5 years old, parents’ education level below high school and family monthly income <3000 yuan were all independent risk factors for CAP scores in children with pre-verbal deafness and cochlear implantation(all P<0.05). Single-factor analysis found that: the age of cochlear implantation, the education level of parents, and the monthly income of families were all the influencing factors of SIR score in pre-verbal deaf cochlear implantation children(all P<0.05).
