摘  要:目的探讨黑龙江省30个地区甲状腺结构异常人群的碘营养与盐碘含量的相关性。方法黑龙江省30个地区(每个地区120名居民)共3 654名成年居民进行调研,根据甲状腺的超声检测结果分为正常组(A组)及异常组,异常组又分为弥漫性病变组(B组)与弥漫性病变伴结节组(C组)、结节组(D组),比较各组间的尿碘及盐碘含量,并对盐碘、尿碘的中位数与甲状腺结构异常的原因进行相关性分析。结果甲状腺正常组与异常组尿碘中位数差异有统计学意义(Z=4.321,P=0.0012);碘缺乏及碘过量均可引起甲状腺患病率升高;四组尿碘中位数处于碘营养适宜水平。甲状腺正常组的碘盐覆盖率、碘盐合格率及合格碘盐食用率均稍高于甲状腺异常组,碘盐覆盖率差异有统计学意义(P<0.05)。结论碘缺乏及碘过量均与甲状腺发病率呈显著相关,碘营养失衡导致黑龙江省甲状腺结构异常的发病率升高。Objective To investigate the correlation between iodine nutrition and salt iodine content in people with abnormal thyroid structure in 30 regions of Heilongjiang Province. Methods A total of 3 654 adult residents in 30 regions of Heilongjiang Province(120 residents in each region) were investigated. According to the ultrasound test results of the thyroid, they were divided into normal group(group A) and abnormal group, and the abnormal group was divided into diffuse lesions group(group B), diffuse lesions with nodules group(group C) and nodule group(group D), the urine iodine and salt iodine contents between each group were compared, the correlation between the median of salt iodine and urine iodine and the cause of thyroid structure abnormality was analyzed. Results The median urinary iodine difference between the normal thyroid structure group and the abnormal group was statistically significant(Z=4.321, P=0.0012);Both iodine deficiency and iodine excess increased the prevalence of thyroid disease;The median of urinary iodine in the four groups was at the appropriate level of iodine nutrition. The coverage rate of iodized salt, the qualified rate of iodized salt, and the rate of qualified iodized salt in the normal thyroid group were slightly higher than those of the abnormal thyroid structure group, and the difference in the coverage of iodized salt was statistically significant(P<0.05). Conclusion Iodine deficiency and iodine excess are significantly related to the incidence of thyroid. Imbalance of iodine nutrition leads to an increase in the incidence of abnormal thyroid structure in Heilongjiang Province.
