乙肝患者血清lncRNA NEAT 1表达与HBV-DNA载量、肝功及T淋巴细胞亚群的相关性

摘　　要：目的分析乙型肝炎(乙肝)患者血清长链非编码RNA核富集转录体1(lncRNA NEAT1)表达与乙型肝炎病毒(HBV)DNA载量、肝功能及T淋巴细胞亚群的相关性。方法按照血清HBV-DNA低、中、高载量1∶1∶1的比例收集2018年2月-2019年9月山东省千佛山医院诊治的慢性乙肝(CHB)患者324例,其中低、中和高载量组各108例。另取同期HBV携带者108例为对照组。采用实时荧光定量PCR(qRT-PCR)检测血清lncRNA NEAT1水平及HBV-DNA载量,采用化学发光法检测血清谷丙转氨酶(ALT)和谷草转氨酶(AST)水平,采用流式细胞仪检测外周血CD3^(+)、CD4^(+)、CD8^(+)细胞水平,计算CD4^(+)/CD8^(+)值。采用Pearson法分析lncRNA NEAT1与血清肝功能指标水平及T淋巴细胞亚群的相关性。结果与对照组比较,低、中、高HBV-DNA载量CHB患者血清lncRNA NEAT1、ALT、AST水平及CD8^(+)细胞比例均显著增加(均P<0.05),CHB患者外周血CD3^(+)、CD4^(+)细胞比例及CD4^(+)/CD8^(+)值均显著降低(均P<0.05)。Pearson分析显示,CHB患者血清lncRNA NEAT1水平与ALT、AST、CD8^(+)细胞比例呈正相关(r分别为0.773、0.761、0.683,均P<0.05),与CD3^(+)、CD4^(+)细胞及CD4^(+)/CD8^(+)值呈负相关(r分别为-0.598、-0.719、-0.770,均P<0.05)。结论CHB患者血清lncRNA NEAT1水平随HBV-DNA载量增加而升高,且与肝功能异常及炎症反应有关,检测血清lncRNA NEAT1可能对CHB患者病情进展及发病机制研究有一定指导价值。Objective To analyze the relationship between the expression of lncRNA nuclear enriched abundant transcript 1(lncRNA NEAT1)and the hepatitis b virus(HBV)DNA viral load,liver function,and T lymphocyte subsets in patients with hepatitis B.Methods Subjects were 324patients with chronic hepatitis B(CHB)seen at Shandong Qianfoshan Hospital from February 2018to September 2019.Patients with a low,medium,or high HBV-DNA load were selected at a ratio of 1:1:1.There were 108patients in each group(low,medium and high HBV-DNA load),and 108patients in the control group.The level of serum lncRNA NEAT1and the HBV-DNA load were detected using quantitative real-time PCR(qRT-PCR),the levels of serum alanine aminotransferase(ALT)and aspartate aminotransferase(AST)were measured using chemiluminescence,the levels of CD3^(+),CD4^(+)and CD8^(+)T lymphocyte subsets in peripheral blood were detected using flow cytometry,and the CD4^(+)/CD8^(+)value was calculated.Pearson correlation analysis was used to analyze the correlation between lncRNA NEAT1and serum liver function and T lymphocyte subsets.Results The levels of serum lncRNA NEAT1,ALT,AST,and the proportion of CD8^(+)cells in patients with CHB and a low,medium,or high HBV-DNA load were significantly higher than those in the control group(P<0.05for all),while the proportions of peripheral blood CD3^(+)and CD4^(+)cells and the CD4^(+)/CD8^(+)cell ratio in patients with CHB were significantly lower(P<0.05for all).Pearson analysis indicated that serum lncRNA NEAT1level was positively correlated with ALT,AST,and the proportion of CD8^(+)cells in patients with CHB(r=0.773,0.761,and 0.683,respectively,P<0.05for all),and negatively correlated with the proportions of CD3^(+)and CD4^(+)cells and the CD4^(+)/CD8^(+)cell ratio(r=-0.598,-0.719,-0.770,respectively,P<0.05for all).Conclusion The level of serum lncRNA NEAT1in patients with CHB increased with an increase in the HBV-DNA load,and that increase was associated with liver dysfunction and an inflammatory reaction.The detection of serum lnc
