重庆市健康儿童类胰岛素生长因子-1及类胰岛素生长因子结合蛋白-3水平研究

摘　　要：目的建立重庆市7~18岁健康儿童类胰岛素生长因子-1(IGF-1)及类胰岛素生长因子结合蛋白-3(IGFBP-3)正常值,并探讨其与年龄、性别、身高、体重、青春发育分期的关系。方法2015年10月至2016年1月选取重庆市6所大、中、小学7~18岁健康儿童1024名(男477名,女547名),测量并记录其年龄、性别、身高、体重、青春发育分期,采用化学发光法测定空腹血清IGF-1、IGFBP-3水平。分析IGF-1、IGFBP-3与其年龄、性别、身高、体重、青春发育分期的关系。结果从学龄期进入青春期,男、女孩IGF-1及IGFBP-3水平随年龄或青春发育分期增长均呈先升高,达高峰后进入平台期,后稍下降维持于稍低于平台期位置的趋势。女孩比男孩提前1~2岁/1~2个青春发育分期进入IGF-1、IGFBP-3高峰期。身高及年龄对IGF-1的影响较大,性别、发育阶段及体重对IGF-1的影响较小。结论本研究结果与国内外其他研究结果有所差异,建立了重庆市7~18岁健康儿童IGF-1、IGFBP-3正常值。Objective The normal values of insulin-like growth factor-1(IGF-1)and insulin-like growth factor-binding protein-3(IGFBP-3)in healthy children aged 7-18 in Chongqing were established by investigating growth and adolescence in normal children in Chongqing and testing blood samples.And explore their relationship with age,gender,height,weight,and stage of puberty development.Methods A total of 1024 healthy children aged 7-18 years(477 males and 547 females)from 6 primary and secondary schools in Chongqing were selected and their age,sex,height,weight,and stage of puberty development were measured and recorded.Fasting serum IGF-1 and IGFBP-3 levels were measured by chemiluminescence.The relationship between IGF-1,IGFBP-3 and age,sex,height,weight,and stage of puberty development were studied by t test and multiple regression analysis.Results From school age to puberty,the levels of IGF-1 and IGFBP-3 in boys and girls showed a first increase with the growth of the age or stage of pubertal development,reached the plateau after reaching the peak,and then slightly decreased and remained at a position slightly lower than the plateau position.In the same age group/stage of puberty development,serum IGF-1 and IGFBP-3 were different between boys and girls,and the girl entered the peak stage of IGF-1 and IGFBP-3 about 1 to 2 years of age/1 to 2 stages of pubertal development in advance of the boy.Height and age have a greater impact on IGF-1 values.Gender,developmental stage,and body weight have less influence on IGF-1 values.Conclusion The results of this survey are different from other surveys at home and abroad.The normal values of IGF-1 and IGFBP-3 for healthy children aged 7-18 in Chongqing were established.
