AngⅡ、IL-6和TNF-α与类风湿关节炎动脉粥样硬化相关性的临床研究

摘　　要：目的探讨血管紧张素Ⅱ(AngⅡ)、白细胞介素-6(IL-6)和肿瘤坏死因子-α(TNF-α)与类风湿关节炎动脉粥样硬化(AS)相关性的,从而找到新的预测类风湿关节炎心血管风险的指标。方法2017-2019年在该院住院的63例类风湿关节炎(RA)患者设为研究组,选择20例同期体检的体检健康者设为对照组。采用GE Vivid-7彩色超声诊断仪,探头频率4~10 MHz。对两组的AngⅡ、IL-6、TNF-α和颈总动脉IMT进行对比分析。AngⅡ测定为卧位:正常值为23~75 pg/mL。结果类风湿关节炎组AngⅡ、IL-6、TNF-α水平均高于对照组,同时AngⅡ、IL-6、TNF-α越高,B超测定双侧颈总动脉中层内膜厚度(IMT)值越高,提示AngⅡ、IL-6、TNF-α越高的RA患者发生心血管疾患的概率就越高。结论AngⅡ、IL-6、TNF-α与动脉粥样硬化成正相关,故可能成为新的预测心血管疾病风险的指标。Objective To investigate the expression of angiotensinⅡ(AngⅡ),interleukin-6(IL-6)and tumor necrosis factor-α(TNF-α)and atherosclerosis in rheumatoid arthritis(AS),In order to find new indicators to predict the cardiovascular risk of rheumatoid arthritis.Methods Sixty-three rheumatoid arthritis(RA)patients hospitalized in the Department from 2017 to 2019 were set as the study group,and 20 healthy patients with physical examination at the same time were selected as the control group.The frequency of the probe was 4-10mhz with GE vid-7 color ultrasonic diagnostic instrument.AngⅡ,IL-6,TNF in two groups-αThe IMT of common carotid artery was compared with that of common carotid artery.AngⅡwas measured as lying position:the normal value was 23-75pg/ml.Results The levels of AngⅡ,IL-6 and TNF-αin RA group were higher than those in the control group.Meanwhile,the higher angⅡ,IL-6 and TNF-αwere,the higher IMT value of bilateral common carotid artery measured by B-ultrasound was,suggesting that RA patients with higher angⅡ,IL-6 and TNF-αwere more likely to have cardiovascular disease.Conclusion AngⅡ,IL-6 and TNF in rheumatoid arthritis group-αThe levels of AngⅡ,IL-6 and TNF were higher than those of the control group-αThe higher the IMT,the higher the level of AngⅡ,IL-6 and TNF-αThe higher the RA,the higher the risk of cardiovascular disease.
