患者小肠中细菌过度生长与NAFLD的关系研究

摘　　要：目的探讨患者小肠内细菌过度生长在非酒精性脂肪肝(NAFLD)中的作用。方法选取2018年1月至2019年12月在我院治疗的NAFLD患者110例(NAFLD组),其中轻度脂肪变患者72例,中重度脂肪变患者38例;同时选取健康志愿者100例作为对照组;两组均进行乳果糖H2呼气试验。结果NAFLD组患者小肠内细菌过度生长发生率为51.82%,明显高于对照组(P<0.05);NAFLD组中重度脂肪变患者小肠内细菌过度生长发生率为71.05%,明显高于轻度脂肪变患者(P<0.05);NAFLD组小肠内细菌过度生长阳性者血清透明质酸(HA)、层黏蛋白(LN)、Ⅲ型前胶原(PCⅢ)和Ⅳ型胶原(Ⅳ-C)分别为(134.40±21.10)ng/ml、(176.61±30.22)ng/ml、(130.22±21.13)ng/ml和(74.40±13.32)ng/ml,明显高于小肠内细菌没有过度生长的患者(P<0.05)。结论NAFLD患者小肠内细菌过度生长发生率高,可能在疾病发生发展中有一定作用。Objective To investigate the role of intestinal bacterial overgrowth in nonalcoholic fatty liver(NAFLD).Methods From January 2018 to December 2019,110 NAFLD patients(NAFLD group)were selected,including 72 patients with mild steatosis and 38 patients with moderate to severe steatosis,and 100 healthy volunteers were selected as the control group;Both groups were given lactulose H2 breath test.Results The incidence of intestinal bacterial overgrowth in the NAFLD group was 51.82%,whhich was significantly higher than that in the control group(P<0.05);The incidence of intestinal bacterial overgrowth in patients with moderate to severe NAFLD was 71.05%,which was significantly higher than that in patients with mild NAFLD(P<0.05);The serum hyaluronic acid(HA)and protein(LN),typeⅢsticking layer before collagenⅢ(PC)and collagen typeⅣ(Ⅳ-C)in intestinal bacterial overgrowth positive patients in NAFLD group were(134.40±21.10)ng/ml,(176.61±30.22)ng/ml,(130.22±21.13)ng/ml and(74.40±13.32)ng/ml,which were significantly higher than the intestine bacterial overgrowth negative(P<0.05).Conclusion The incidence of intestinal bacterial overgrowth in patients with NAFLD is high,which may play a role in the occurrence and development of the disease.
