摘  要:目的探讨早期程序化康复训练对重症加强护理病房(Intensive Care Unit,ICU)机械通气患者预后的影响。方法选取2019-02至2020-05入住我院ICU符合入选标准的患者80例随机数字表法分为对照组(40例)和观察组(40例),对照组按照ICU护理常规及肢体功能锻炼,观察组在常规护理的基础上,对入科72 h内患者给予制定程序化的康复计划进行康复训练。比较两组患者压力性损伤发生率、深静脉血栓发生率、ICU获得性衰弱(Intensive Care Unit Acquired Weakness,ICUAW)发生率、日常生活能力评定(Barthel)指数、机械通气时间、拔管成功率、ICU住院时间。结果与对照组比较,观察组患者压力性损伤发生率差异无统计学意义(2.5%比15%,P>0.05);观察组患者的深静脉血栓发生率(0%比10%)、ICU-AW发生率(22.5%比52.5%)、Barthel指数[(34.88±11.27)比(28.13±11.25)]优于对照组差异有统计学意义(P<0.05)。观察组患者的机械通气时间[(12.45±3.21)d比(14.33±3.14)d]、拔管成功率(37.5%比17.5%)、ICU住院时间[(21.15±6.81)d比(27.28±8.45)d]优于对照组,差异有统计学意义(P<0.05)。结论采用早期程序化康复训练可缩短住院时间,提高病人自理能力,减少相关并发症的发生,具有推广价值。Objective To explore the effects of early programmed rehabilitation training on mechanical ventilation patients in ICU.Methods According to the inclusion criteria,a total of 80 patients given mechanical ventilation in ICU admitted to our hospital from February 2019 to May 2020 were randomly divided into the control group(40 cases)and the observation group(40 cases).The control group was treated with ICU nursing routine and limb function exercise,while the observation group was given rehabilitation training within 72 hours of admission apart from the treatment adopted by the control group.The differences between the two groups were observed in the incidence of pressure injury,deep venous thrombosis,Intensive Care Unit Acquired Weakness(ICU-AW),Barthel index,mechanical ventilation time,extubation success rate,and ICU hospital stays.Results Compared with the control group,the incidence of pressure injury in the observation group had no significant difference(2.5%vs 15%,P>0.05);The observation group was significantly better than that in the control group(P<0.05)in the incidence of deep venous thrombosis(0%vs 10%),the incidence of ICU-AW(22.5%vs 52.5%),the Barthel index[(34.88±11.27)vs(28.13±11.25)],the mechanical ventilation time[(12.45±3.21)d vs(14.33±3.14)d],the success rate of extubation(37.5%vs 17.5%),and the length of stay in ICU[(21.15±6.81)d vs(27.28±8.45)d].Conclusion Early programmed rehabilitation training could shorten the hospital stay,improve patients'self-care ability,and reduce the incidence of related complications,which should be recommended and applied widely.
