磁共振弹性成像测量脑血管生物力学特性研究

摘  要:目的:探讨脑血管生物力学特性的测量,为脑血管疾病诊断提供重要信息。方法:于2018年至2020年共招募61例腔隙性脑梗死患者和63名健康志愿者,均行3T磁共振扫描,分别获取磁共振弹性成像(MRE)和磁共振血管成像(MRA)的图像数据。在MRA图上选取感兴趣区,对脑血管进行分割,并在MRE和MRA图像配准后在MRE上绘制血管层面图,再计算脑血管的储能模量和绝对剪切模量,从而得到脑血管的生物力学特性。年龄、性别和腔隙性脑梗死对脑血管生物力学特性的影响采用Pearson相关性分析、Student t检验和方差分析进行统计学处理。结果:腔隙性脑梗死组的绝对剪切模量与健康对照组相比明显降低(P=0.02)。性别差异方面与脑血管绝对剪切模量之间没有显著的相关性(P=0.39)。年龄差异方面,综合样本数据进行分析,绝对剪切模量随年龄的增加而减小(P=0.02)。结论:脑血管的生物力学特性与脑梗死疾病有密切相关性。脑血管特性的体内测量可为脑梗死的表征和诊断提供更多实用价值。Purpose:To explore the biomechanical characteristics of cerebral vessels and to provide important information for diagnosis of diseases.Methods:A total of 61 patients with lacunar infarction and 63 healthy volunteers were recruited between 2018 and 2020.Both magnetic resonance elastography(MRE)and magnetic resonance angiography(MRA)were performed for each patient on a 3T scanner.Cerebral vessels were segmented on MRA and were mapped out on MRE after registration of MRE and MRA images.The storage and absolute shear moduli of cerebral vessels was estimated.Effects of age,sex,and lacunar infarction on the biomechanical properties of the cerebral vessels were analyzed using Pearson correlations,Student t-tests,and ANOVA.Results:The absolute shear moduli of the cerebral vessels were significantly lower in the lacunar infarction group than those in the healthy control group(P=0.02).There was no significant correlation between gender differences and absolute cerebrovascular shear modulus(P=0.39).In terms of age difference,the absolute shear modulus decreases with the increase of age(P=0.02).Conclusion:The study showed that the biomechanical properties of cerebral vessels were closely related with the infarction disease.Our results indicated that in vivo measurements of the tissue properties could provide more perspectives for cerebral infarction characterization and diagnosis.
