摘  要:目的:通过MR血氧水平依赖成像(BOLD)初步评价兔肾脏铁过载病理变化的可行性。方法:将24只纯种健康新西兰大白兔随机分为对照组(n=12)和铁过载组(n=12)。铁过载组兔经后腿肌内注射右旋糖酐铁(60 mg/kg),对照组后腿肌内注射同等剂量0.9%生理盐水,分别于建模后即刻(0周)及第12周对2组兔行左肾MR扫描,12周扫描结束后立即切除左肾行病理学检查。在T_(2)^(*)map图像上分别测量计算肾皮质、外髓的R_(2)^(*)值及R_(2)^(*)外髓/皮质比值(MCR)。采用Mann-Whitney U检验比较肌内注射后0周及第12周对照组与铁过载组之间R_(2)^(*)值的差异;采用Wilcoxon检验分别比较对照组及铁过载组实验兔肌内注射后0周与第12周R_(2)^(*)值的差异。结果:肌内注射后0周时,对照组与铁过载组之间皮髓质R_(2)^(*)值及MCR值差异无统计学意义(P 0.05)。肌内注射后第12周时,铁过载组皮质、外髓R_(2)^(*)值显著高于对照组(P 0.05),MCR值显著低于对照组(P 0.05)。铁过载组第12周肾皮质及髓质R_(2)^(*)值较0周增高,MCR值较0周减低(P 0.05)。结论:铁过载时兔肾皮质R_(2)^(*)值的变化较外髓R_(2)^(*)变化更为明显:兔肾皮质及外髓R_(2)^(*)值增高,MCR值减低。BOLD评价兔铁过载介导肾损伤的病理变化具有可行性。Purpose:To evaluate the feasibility of the study of pathological changes of renal iron overload in rabbits by blood oxygen level-dependent MR imaging.Methods:Twenty-four healthy New Zealand rabbits were randomly divided into iron overload group(n=12)and control group(n=12).The iron overload group was injected with iron-dextran(60 mg/kg);The control group was injected with the same amount of 0.9%normal saline in the same way.All the rabbits underwent MR examinations of the left kidney immediately(0w)and 12 weeks(12w)after modeling.After the examinations,nephrectomy was performed for histologic evaluation.The T_(2)^(*)relaxation rate of the renal cortex(CR_(2)^(*))and the outer medulla(MR_(2)^(*))were measured separately,and then the value of MCR was calculated.Mann-Whitney U test was used to compare the difference of R_(2)^(*)values between the two groups at 0 and 12 weeks,while the CR_(2)^(*),MR_(2)^(*),MCR of 0w and 12w in the two groups was compared respectively by using Wilcoxon test.Results:The CR_(2)^(*)and MR_(2)^(*)values of the iron overload group at 12w were higher than those of the control group,and the value of MCR were lower(P<0.05),while those R_(2)^(*) values in two groups at Ow were with no significant difference(F>0.05).The CR_(2)^(*) values and the MR_(2)^(*) values of the iron overload group at 12w were higher than those at 0w,and the value of MCR were lower(P<0.05).Conclusion:The change of R_(2)^(*) value in renal cortex was more obvious than that in outer medulla,the R_(2)^(*) value of renal cortex and outer medulla increased and the MCR value decreased during iron depostion.BOLD has certain value for evaluating the pathological development of iron mediated renal injury in rabbits.
