三维超声造影诊断中弓韧带压迫综合征的价值

摘　　要：目的探讨三维超声造影诊断中弓韧带压迫综合征的价值。方法对彩色多普勒超声筛查考虑中弓韧带压迫综合征的患者进行三维超声造影检查,并与CTA结果进行对比分析。使用SPSS 25.0软件计算诊断的灵敏度、特异度和两种影像学方法之间的一致性Kappa值,评价三维超声造影诊断的可靠性。结果 26例患者成功进行三维超声造影,检出阳性18例,阴性8例。阴性患者腹腔动脉内径随呼吸状态无明显变化。阳性患者表现为深呼气末腹腔动脉造影剂束呈特征性钩状狭窄甚至中断,深吸气末造影剂束恢复正常。全部患者均经CTA检查,证实超声造影结果中假阳性1例。三维超声造影灵敏度100%(17/17)、特异度89%(8/9)、准确度96%(25/26)、Kappa值0.91。结论三维超声造影可清晰地显示腹腔动脉特征性钩状狭窄和随呼吸变化的动态过程,为中弓韧带压迫综合征的诊断提供了一种新方法。Objective To approach the value of threedimentional contrast enhanced ultrasonography(3 D-CEUS) in the diagnosis of median arcuate ligament syndrome(MALS). Methods The patients suspected as MALS after screening by CDFI, then 3 D-CEUS were performed, and contrasted with CT angiography(CTA).Software SPSS 25.0 was used to calculate sensitivity, specificity, and Kappa value, and evaluate the reliability of 3 D-CEUS. Results Total 26 cases were achieved by 3 D-CEUS, and 18 positive and 8 negative cases were revealed. In the negative group, contrast beam didn′t change with respiration.In positive group the characteristic findings revealed that contrast beam became hooked-like stenosis or interrupted at end-expiration and returned normal at end-inspiration. All the cases were confirmed by CTA, 1 false-positive were determined. The sensitivity of 3 D-CEUS was 100%, the specificity, 89%, the accuracy, 96%, and Kappa value, 0.91. Conclusions 3 D-CEUS can accurately reveal the characteristic hooked-likestenosis and the dynamic changes with respiration, whichis in line with CTA excellently and offers a new approach for the diagnosis of MALS.
