摘  要:目的分析导致胎盘早剥孕妇新生儿颅脑超声结果阳性的危险因素。方法收集因胎盘早剥住院生产的54例孕妇及其新生儿临床资料,将新生儿颅脑超声检查结果分为阳性组及阴性组,分析导致阳性结果的相关危险因素,并对比新生儿结局与颅脑超声结果的相关性。结果 (1)54例新生儿中颅脑超声阳性组占51.9%(28/54),阴性组占48.1%(26/54),对比两组临床资料,阳性组胎儿娩出孕周小于阴性组,差异有统计学意义(P<0.05);孕妇合并子痫前期,两组比较差异有统计学意义(P<0.05);而孕妇是否合并妊娠期糖尿病、甲状腺功能减低、胎膜早破及胎盘剥离的轻重程度,两组差异均无统计学意义(P>0.05)。(2)根据胎儿娩出时间将新生儿分为早产儿组及足月儿组,早产儿组颅脑超声阳性高于足月儿组,差异有统计学意义(P<0.05)。(3)全部54例新生儿中合并新生儿窒息或新生儿呼吸窘迫综合征(neonatal respiratory distress syndrome, NRDS)者11例,占20.4%,其中颅脑超声阳性者10例,占90.9%(10/11);与未出现上述并发症的43例新生儿中的18例(41.9%)颅脑超声阳性者比较,差异有统计学意义(P<0.05)。结论早产及存在窒息或NRDS并发症与胎盘早剥孕妇新生儿中颅脑超声结果阳性相关,应用颅脑超声可早期诊断胎盘早剥孕妇新生儿颅脑损伤,有助于改善预后。Objective Analysis of risk factors leading to positive results of brain ultrasound in newborns of pregnant women. Methods Collect clincal date of 54 pregront wormen and their newborns who were hospital due to placental obruption, divide the neonatal craniocerebral ultrasound results into positive and negative groups, and analyze the related risks that lead to positive results factors, and compare the correlation between neonatal outcomes and brain ultrasound results. Results(1)Among 54 newborns, the positive group of brain ultrasound, accounting for 51.9%(28/54), and the negative group, accounting for 48.1%(26/54). Compared with the clinical data of the two groups, the positive group had less gestational weeks than the negative group, with statistically significant difference( P<0.05);For pregnant women with preeclampsia, the difference between the two groups was statistically significant(P<0.05);while for pregnant women with gestational diabetes, hypothyroidism, or whether there was premature rupture of membranes and placental ablation, there was no difference between the two groups Statistical significance(P>0.05).(2)Newborns were divided into premature and full-term groups according to the time of delivery of the fetus. The positive rate of craniocerebral ultrasound in the premature group was higher than that in the full-term group, with statistically significant difference(P< 0.05).(3)Among all 54 newborns, 11 cases were combined with neonatal asphyxia or neonatal respiratory distress syndrome(Neonatal respiratory distress syndrome, NRDS), accounting for 20.4%, of which 10 cases were positive for brain ultrasound, accounting for 90.9%(10/11);Compared with 18 cases(41.9%) of 43 newborns without the above complications, the difference was statistically significant(P<0.05). Conclusions Premature delivery and the presence of asphyxia or NRDS complications are associated with positive brain ultrasound results in pregnant women with placental abruption.
