摘  要:目的应用产前超声观察不同孕周男性胎儿尿道海绵体部线样回声的形态及其长度,探索各测值与孕周的关系,以期能够辅助诊断胎儿尿道下裂。方法收集285例孕周为19~37周的正常男性胎儿。使用二维超声生殖器矢状面测量正常男性胎儿尿道海绵体部长度,三次测量取平均值。当胎儿外生殖器形态异常,可疑尿道下裂时,观察胎儿尿道线样回声能否达到阴茎末端,同时测量尿道长度。应用SPSS 22.0软件统计分析。结果男性胎儿尿道海绵体部长度均随孕周变化,呈线性相关(R^(2)=0.92,P<0.001),经线性回归分析可建立线性方程为:Y(mm)=2.0X-20.2(孕周为自变量X,胎儿尿道海绵体部长度为因变量Y)。可疑尿道下裂胎儿尿道线样回声中断,未达阴茎末端,同时海绵体部尿道长度低于95%可信区间。结论男性胎儿的海绵体部尿道长度与孕周呈线性相关。海绵体部尿道的观察和测量有助于诊断尿道下裂。Objective Prenatal ultrasound was used to observe the shape and length of the line-like echo of fetal urethral cavernous body in different gestational weeks.In order to be able to assist in the diagnosis of fetal hypospadias. Methods 285 normal male fetuses with 19-37 weeks of gestation were collected. the normal male fetal urethral cavernous body length was measured using two-dimensional ultrasound genital sagittal plane, and the mean value was taken three times. When the shape of the external genitalia of the fetus was abnormal and the hypospadias were suspected, the fetal urethral line-like echo can reach the end of the penis, and the length of the urethra was measured at the same time. applied SPSS 22.0 software statistical analysis. Results The length of the urethral cavernous part of the male fetus was linearly correlated with the gestational week(R2=0.92, P=0.001). the linear equation can be established by linear regression analysis: Y(mm)=2.0 X-20.2(gestational week is independent variable X, fetal urethral cavernous length was dependent variable). Suspected hypospadias foetal urethral line-like echo was interrupted and did not reach the end of the penis, while the length of the urethra in the cavernous part was less than 95% confidence interval. Conclusions The length of the urethra in the male fetus is linearly correlated with the gestational week. Observation and measurement of the cavernous part of the urethra is helpful for the diagnosis of hypospadias.
