以头面部肿胀为首发症状的上腔静脉综合征

摘　　要：报道1例上腔静脉综合征。患者男,64岁。头面部肿胀2个月余。皮肤科检查:双侧眼睑明显肿胀,睁眼困难,头面部及颈部弥漫性肿胀,为非指凹性。右胸部、胸骨区域及双乳房内侧可见紫蓝色迂曲扩张的静脉。胸部计算机体层成像(CT)平扫+强化检查:纵膈内肿物侵及邻近上腔静脉,考虑恶性病变可能性大。诊断:上腔静脉综合征。A case of superior vena cava syndrome is reported. A 64-years-old male suffere d he ad and face swelling for more than 2 months. Dermatology examination revealed bilateral eyelids were obviously swollen, and difficult to open. The patient′s head, face and neck were diffusely swollen, which were non-finger concave. Purplish blue tortuous dilated veins were observed in the right upper chest, sternal area, and medial side of both breasts. Chest CT scan and enhancement showed that the mediastinal mass invaded the adjacent superior vena cava, and the lesion was possibly malignant. The diagnosis of superior vena cava syndrome was made.
