摘  要:目的研究肺结核患者医院内感染的相关因素和耐药性分析,为控制和预防院感提供理论依据。方法回顾性分析本院2018年3月-2020年3月的600例肺结核患者基本特征。患者基线特征的比较使用χ^(2)检验,多因素分析采用多元Logistic回归分析。结果肺结核患者医院内感染率为5.00%,糖尿病、结核病史和恶性肿瘤是患者医院内感染的主因,医院内感染率分别为4.91%、4.31%、4.12%。肺结核患者医院内感染的病原菌以革兰阴性菌为主,占56.67%,大肠埃希菌的医院内感染率为26.67%。异烟肼和链霉素的单独耐药率分别为4.00%、8.00%,合并耐药率为10.67%。对利福平和乙胺丁醇的耐药率分别为1.00%、1.67%。结论对于肺结核合并糖尿病、长病史和恶性肿瘤的患者应该加强医院内感染的防控措施,避免单用抗结核药,尤其是异烟肼和链霉素。Objective To study the related factors and drug resistance of nosocomial infection in patients with pulmonary tuberculosis,so as to provide theoretical basis for the control and prevention of nosocomial infection.Methods The basic characteristics of 600 patients with pulmonary tuberculosis in our hospital from March,2018 to March,2020 were analyzed retrospectively.The baseline characteristics of the patients were compared by χ^(2) test,and multivariate Logistic regression analysis was used in multivariate analysis.Results The hospital-induced rate of tuberculosis patients was 5.00%.The incidence of nosocomial infection was respectively 4.91%,4.31% and 4.12% for diabetes mellitus,tuberculosis history and malignant tumor.Gram-negative bacteria were the main pathogens of nosocomial infection in tuberculosis patients,accounting for 56.67%,and the infection rate of Escherichia coli was 26.67%.The single drug resistance rates of isoniazid and streptomycin were 4.00%,8.00%,and the combined drug resistance rates were 10.67%.The drug resistance rates to rifampicin and ethambutol were1.00% and 1.67% respectively.Conclusion For patients with tuberculosis complicated with diabetes,long history and malignant tumors,prevention and control measures should be strengthened,so as to avoid single use of anti-tuberculosis drugs,especially isoniazid and streptomycin.
