血浆t-PAIC、D-D、α2-AP水平在评估下肢深静脉血栓抗凝效果中的应用

摘　　要：目的研究血浆组织型纤溶酶原激活物-纤溶酶原激活物抑制剂复合物(t-PAIC)、D-二聚体(D-D)、α2-抗纤溶酶(α2-AP)水平在评估下肢深静脉血栓抗凝效果中的应用价值。方法将2018年1月至2020年2月于郑州大学第一附属医院接受下肢静脉血栓治疗患者214例作为甲组,同期在郑州大学第一附属医院体检健康人员68例作为乙组,比较甲乙两组人员血浆t-PAIC、D-D、α2-AP水平,评估各监测指标诊断下肢深静脉血栓的意义,并对甲组患者进行抗血栓、抗凝治疗,以治疗后彩超复查结果为标准,评估血浆t-PAIC、D-D、α2-AP水平对下肢深静脉血栓抗凝效果的检验效能。结果甲组治疗前t-PAIC、D-D、α2-AP水平高于甲组治疗后及乙组,差异有统计学意义(P<0.05),甲组治疗后t-PAIC、D-D、α2-AP水平与乙组比较,差异未见统计学意义(P>0.05)。t-PAIC检测灵敏度为89.85%、特异度为64.71%、准确度为87.85%;D-D检测灵敏度为77.16%、特异度为47.06%、准确度为74.77%;α2-AP检测灵敏度为81.73%、特异度为58.83%、准确度为79.91%,差异未见统计学意义(P>0.05)。结论血浆t-PAIC、D-D、α2-AP水平检测均可作为判定下肢静脉血栓抗凝治疗效果评估标准,准确性高。Objective To study the application value of tissue plasminogen activator plasminogen activator inhibitor complex(t-PAIC),D-Dimer(D-D)andα2-antithrombin(α2-AP)in evaluating the anticoagulant effect of lower extremity deep venous thrombosis.Methods A total of 214 patients with lower extremity venous thrombosis treated in the First Affiliated Hospital of Zhengzhou University from January2018 to February 2020 were selected as group A,and 68 healthy persons in the First Affiliated Hospital of Zhengzhou University during the same period were selected as group B.The plasma levels of t-PAIC,D-D andα2-AP were compared between the two groups,and the significance of each monitoring indexes in the diagnosis of lower extremity deep venous thrombosis were evaluated.Antithrombotic and anticoagulant therapy was given for the patients in group A.The results of color Doppler ultrasound reexamination were used as the standard to evaluate the test efficacy of plasma t-PAIC,D-D andα2-AP levels on anticoagulant effect of lower extremity deep venous thrombosis.Results The levels of t-PAIC,D-D andα2-AP in group A before treatment were higher than those in group A after treatment and group B,and the differences were significant(P<0.05).The levels of t-PAIC,D-D andα2-AP in group A after treatment were not significantly different from those in group B(P>0.05).The sensitivity,specificity and accuracy of t-PAIC were 89.85%,64.71%and 87.85%respectively;the sensitivity,specificity and accuracy of D-D were 77.16%,47.06%and 74.85%respectively;the sensitivity,specificity and accuracy ofα2-AP were81.73%,58.83%and 79.91%respectively,with no significant difference(P>0.05).Conclusions Plasma t-PAIC,D-D andα2-AP levels can be used as the evaluation standard of anticoagulant treatment effect of lower extremity venous thrombosis with high accuracy.
