不同年龄段患者无创超声心输出量监测的准确性评价

摘　　要：目的探讨评估无创超声心输出量监测技术(USCOM)应用于不同年龄段患者每搏量(SV)、心输出量(CO)监测的准确性。方法将患者按照年龄分为青年组(20~<40岁)、中年组(40~<60岁)、老年组(60~80岁),同时每组分别予以USCOM和经胸二维超声心动图(TTE)测量SV及CO,以TTE为对照。结果3个年龄阶段分别用TTE和USCOM测得的C0与SV比较,差异均无统计学意义(P>0.05)。青年组分别用TTE和USCOM法测得SV平均差值为-2.56 mL,95%置信区间(95%CI)为-14.03~8.92。青年组TTE和USCOM法测得CO平均差值为-0.13 L/min,95%CI为-1.1~0.84。中年组TTE和USCOM法测得SV平均差值为-3.0 mL,95%CI为-18.49~12.49。中年组TTE和USCOM法测得CO平均差值为-0.16 L/min,95%CI为-1.31~0.94。老年组TTE和USCOM法测得SV平均差值为-2.74 mL,95%CI为-19.49~14.55。老年组TTE和USCOM法测得CO平均差值为-0.18 L/min,95%CI为-1.36~1.00。结论不同年龄段的患者应用USCOM和TTE测患者的SV、CO一致性较好,USCOM适用于不同年龄段患者血流动力学的监测。Objective To explore and evaluate the accuracy of non-invasive ultrasound cardiac output monitoring technology(USCOM)applied to the monitoring of stroke volume(SV)and cardiac output(CO)in patients of different ages.Methods The patients were divided into youth group(20~<40 years old),middle-aged group(40~<60 years old),and elderly group(60~80 years old)according to their ages.At the same time,each group was given USCOM and transthoracic two-dimensional echocardiography.(TTE)SV and CO were measured,and transthoracic two-dimensional echocardiography was used as the control.Results There was no statistically significant difference between C0 and SV measured by TTE and USCOM in the three age stages(P>0.05).In the youth group,the average difference in SV measured by TTE and USCOM was-2.56 mL,the 95%CI was-14.03-8.92.The average difference in CO measured by TTE and USCOM in the youth group was-0.13 L/min,and the 95%CI was-1.1-0.84.In the middle-aged group,the average difference in SV measured by TTE and USCOM was-3.0 mL,and the 95%CI was-18.49-12.49.In the middle-aged group,the average difference in CO measured by TTE and USCOM was-0.16 L/min,and the 95%CI was-1.31-0.94.The average difference in SV measured by TTE and USCOM in the elderly group was-2.74 mL,and the 95%CI was-19.49-14.55.The average difference of CO measured by TTE and USCOM in the elderly group was-0.18 L/min,and the 95%CI was-1.36-1.00.Conclusion The consistency of SV and CO measured by USCOM and TTE for patients of different age groups is better.USCOM is suitable for hemodynamic monitoring of patients of different age groups.
