摘　　要：“互联网+”社区慢性病管理涉及政府、卫生行政部门、社区卫生服务机构、三级医院、社区慢性病患者、互联网企业、医药企业、保险公司和科研机构等多方利益主体。在明确各方利益诉求的基础上,深入分析“互联网+”社区慢性病管理存在的问题,并对各利益相关者的角色、现状、利益诉求、利益相关程度、影响力、立场和受政策影响程度等方面展开探讨,得出“互联网+”社区慢性病管理体系与社区卫生服务中心、慢性病患者、政府及卫生行政管理部门等的利益高度相关。最后构建利益激励机制、制约机制、保障机制和协调机制,形成“互联网+”社区慢性病管理的运行机制。“Internet+”community chronic disease management involves multiple interested parties,including government,health administration departments of chronic disease at community level,community health service centers,tertiary hospitals,chronic disease patients in community,Internet companies,pharmaceutical companies,insurance companies and scientific research institutions,etc.On the basis of clarifying interest demands of all parties,the problems existing in“Internet+”community chronic diseases management was comprehensively analyzed,and the aspects like the roles,current situation,interests demands,interests relevant degree,influence,standing point and vulnerability to policy of interested parties were discussed as well.Thus,the conclusion that“Internet+”community chronic disease management system is highly correlated with the interests of community health service centers,chronic disease patients in community,government and the health administration departments of chronic disease at community level was drawn.Finally,building mechanisms of benefit motivation,restriction,safeguard and coordination was proposed to form an operation mechanism of“Internet+”community chronic disease management.
