摘 要:目的探究纳洛酮与氨茶碱对小儿急性呼吸衰竭的疗效及对电解质水平的影响。方法选择2019年3月—2020年5月期间该院收治82例急性呼吸衰竭患儿,根据抽签法进行随机分组,对照组(常规治疗)和观察组(纳洛酮联合氨茶碱治疗)各41例。比较两组患儿治疗前后电解质水平、血气分析和临床治疗总有效率。结果治疗后观察组电解质水平钾、钠、钙、氯正常率(92.68%、95.12%、97.56%、90.24%)高于对照组(58.54%、56.09%、63.41%、53.66%),PaO2(72.62±2.43)mmHg、SaO2(95.37±2.54)%及pH(7.62±0.11)高于对照组(62.45±3.12)mmHg、(86.42±3.25)%,PaCO2(48.35±2.26)mmHg低于对照组,差异有统计学意义(χ^(2)=12.961、16.929、15.219、13.596,t=16.467、13.894、12.307、24.147,P<0.05);观察组患儿临床治疗总有效率(92.68%)高于对照组70.73%,差异有统计学意义(χ^(2)=6.609,P<0.05)。结论小儿急性呼吸衰竭行纳洛酮联合氨茶碱治疗能够有效改善患儿异常的电解质水平,改善其血气分析,提升临床疗效。Objective To explore the effect of naloxone and aminophylline on children with acute respiratory failure and their influence on electrolyte levels.Methods From March 2019 to May 2020,82 children with acute respiratory failure were selected and treated in the hospital.They were randomly divided into groups according to the lottery method,the control group(conventional treatment)and the observation group(naloxone combined with aminophylline treatment),41 cases each.The electrolyte levels,blood gas analysis and total effective rate of clinical treatment were compared between the two groups of children before and after treatment.Results After treatment,the normal rate of electrolyte levels of potassium,sodium,calcium and chlorine in the observation group(92.68%,95.12%,97.56%,90.24%)was higher than that of the control group(58.54%,56.09%,63.41%,53.66%),PaO2(72.62±2.43)mmHg,SaO2(95.37±2.54)%and pH(7.62±0.11)were higher than the control group by(62.45±3.12)mmHg,(86.42±3.25)%,PaCO2(48.35±2.26)mmHg,lower than the control group.The difference was statistically significant(χ^(2)=12.961,16.929,15.219,13.596,t=16.467,13.894,12.307,24.147,P<0.05);the total effective rate of clinical treatment of children in the observation group(92.68%)was higher than that of the control group by 70.73%,the difference was statistically significant(χ^(2)=6.609,P<0.05).Conclusion Naloxone combined with aminophylline treatment in children with acute respiratory failure can effectively improve the abnormal electrolyte levels of children,improve their blood gas analysis,and improve clinical efficacy.Therefore,it can be used for clinical promotion.
