B超诊断小儿先天性胆总管囊性扩张症的价值分析

摘　　要：目的观察在对小儿先天胆总管囊性扩张症的诊断方式中采用B超诊断的价值分析。方法对该院2019年4月—2020年1月采用B超诊断的30例小儿先天性总管囊性扩张症患者选取,回顾性分析其术前B型超声检查以及内镜下逆行胰胆管造影手术资料,评价B型超声检查在小儿先天性胆总管囊性扩张症诊断效果。结果纳入30例先天性胆总管囊性扩张症患儿经B型超声诊断29例、1例假阴性患儿,诊断准确率达到96.67%;术前ERCP检查确诊30例,确诊率100.00%。B型超声诊断先天性胆总管囊性扩张症与术前ERCP检查差异无统计学意义(P>0.05)。结论采用先天性胆总管囊状扩张症B型超声诊断可清晰显示出先天性胆总管囊状扩张症发生的具体范围、部位、形态与程度,可直观反映出肝内、外组织、大血管与囊状扩张部分胆总管关系,通过B型超声诊断可为此类患者手术治疗与预后提供参考依据。Objective To observe the value analysis of B-ultrasound diagnosis in the diagnosis of children with congenital choledochal cystic dilatation.Methods 30 cases of pediatric congenital common duct cystic dilatation diagnosed by B-ultrasound from April 2019 to January 2020 in the hospital were selected,and their preoperative B-mode ultrasonography and endoscopic retrograde cholangiopancreatography surgical data were retrospectively analyzed to evaluate the diagnostic effect of B-mode ultrasonography in children with congenital cystic dilatation of the common bile duct.Results Thirty children with congenital cystic dilatation of the common bile duct were diagnosed by B-mode ultrasound in 29 cases,and 1 case was false-negative.The diagnosis accuracy rate reached 96.67%;30 cases were diagnosed by preoperative ERCP examination,and the diagnosis rate was 100.00%.There was no statistically significant difference between B-mode ultrasound diagnosis of congenital cystic dilatation of the common bile duct and preoperative ERCP(P>0.05).Conclusion The B-mode ultrasound diagnosis of congenital cystic dilatation of the common bile duct can clearly show the specific scope,location,shape and degree of the occurrence of congenital cystic dilatation of the common bile duct,which can directly reflect the internal and external tissues and large blood vessels of the liver.The relationship between the cystic dilatation part of the common bile duct,B-mode ultrasound diagnosis can provide a reference for the surgical treatment and prognosis of such patients.
