展筋活血方穴位贴敷联合百笑灸治疗寒湿痹阻型膝骨关节炎临床研究

摘　　要：目的观察展筋活血方穴位贴敷联合百笑灸治疗寒湿痹阻型膝骨关节炎(KOA)的临床疗效。方法将70例寒湿痹阻型KOA患者按照随机数字表法分为2组。对照组35例予塞来昔布胶囊口服治疗,治疗组35例在对照组治疗基础上予展筋活血方穴位贴敷联合百笑灸治疗。2组均治疗4周。比较2组治疗前后疼痛视觉模拟评分(VAS)、西安大略和麦克马斯特大学(WOMAC)骨关节炎指数评分及膝关节运动功能Lysholm评分变化,并统计2组疗效。结果治疗组总有效率91.43%(32/35),对照组总有效率77.14%(27/35),治疗组疗效优于对照组(P<0.05)。治疗后2组疼痛VAS均较本组治疗前降低(P<0.05),且治疗后治疗组低于对照组(P<0.05)。治疗后2组WOMAC骨关节炎指数各项评分及总分均较本组治疗前降低(P<0.05),且治疗后治疗组关节疼痛、功能活动评分及总分均低于对照组(P<0.05)。治疗后2组膝关节运动功能Lysholm各项评分均较本组治疗前升高(P<0.05),且治疗后治疗组均高于对照组(P<0.05)。结论展筋活血方穴位贴敷联合百笑灸治疗寒湿痹阻型KOA疗效优于塞来昔布胶囊口服,可有效缓解患者膝关节疼痛,改善关节功能,提高临床疗效。Objective To observe the clinical efficacy of Zhanjin Huoxue Prescription acupoint application combined with Baixiao moxibustion for knee osteoarthritis(KOA)of cold dampness obstruction type.Methods Totally 70 patients with KOA of cold dampness obstruction type were randomly divided into two groups,35 cases per group.The control group was treated with oral celecoxib capsules,and the treatment group was treated with Zhanjin Huoxue Prescription acupoint application combined with Baixiao moxibustion on the basis of the control group.The course of treatment in both groups was 4 weeks.Visual analogue scale(VAS)score,Western Ontario and McMaster University Osteoarthritis Index(WOMAC)score and Lysholm score of knee joint function between the two groups were compared before and after treatment,and the clinical efficacy in the two group was counted.Results The total effective rate in the treatment group was better than that in the control group[91.43%(32/35)vs 77.14%(27/35),(P<0.05),respectively].Compared with before treatment,VAS score,WOMAC score and total scores in both groups were significantly decreased(all P<0.05),which were lower in the treatment group than the control group(all P<0.05);Lysholm score of knee joint function in both groups was significantly increased(P<0.05),which was higher in the treatment group than the control group(P<0.05).Conclusion Zhanjin Huoxue Prescription acupoint application combined with Baixiao moxibustion is better than oral celecoxib capsule in the treatment of KOA of cold dampness obstruction type,which can effectively relieve the pain of knee joint,improve the joint function,and improve the clinical efficacy.
