日间手术对综合医院手术科室平均住院日的影响

摘　　要：目的分析日间手术对综合医院手术科室平均住院日的影响。方法回顾性收集2016年度和2017年度河南省人民医院8个科室(眼科、耳鼻喉科、骨科、泌尿外科、普外科、妇科、血管瘤科)患者信息,比较2016年日间手术实施前与2017年日间手术实施后患者住院情况。结果2017年出院患者性别、年龄及基础疾病情况与2016年差异无统计学意义(P>0.05)。2017年平均住院日[(7.30±2.09)d]较2016年[(8.72±2.56)d]短(P<0.001),出院患者次均费用[(20262.43±10328.19)元]较2016年[(21983.05±11053.83)元]低(P=0.014)。2017年共9444人次接受日间手术,占出院总人次的11.87%。非日间手术平均住院日为(8.15±2.37)d,所有患者总平均住院日为(7.30±2.09)d,较非日间手术平均住院日短0.85 d(P=0.004)。眼科、整形外科、普外科、耳鼻喉科、泌尿外科、血管瘤科日间手术人次分别占出院总人次的32.44%、20.32%、9.83%、8.32%、6.74%、4.95%,平均住院日分别缩短1.55、1.38、0.83、0.53、0.47、0.35 d(P<0.05)。妇科和骨科日间手术人次占出院总人次比率分别为4.41%、1.21%,日间手术对平均住院日影响不大(P>0.05)。平均住院日缩短时间与日间手术人次占出院总人次比率呈正相关(R^(2)=0.921,P=0.003)。结论开展日间手术有助于缩短平均住院日,缩短天数与日间手术人次占出院总人次的比率呈正相关。Objective To analyze the impact of day surgery on the average hospital stay in the operating department of a general hospital.Methods The data on patients from 8 operating departments(ophthalmology,otolaryngology,orthopedics,urology,general surgery,gynecology,hemangioma)in Henan Provincial People’s Hospital in 2016 and 2017 was retrospectively collected.Compare the hospitalization status of patients before the implementation of day surgery in 2016 and after the implementation of day surgery in 2017.Results The gender,age and underlying diseases of discharged patients in 2017 were not statistically different from those in 2016(P>0.05).The average hospital stay in 2016 and 2017 were(8.72±2.56)days and(7.30±2.09)days,respectively.The average hospital stay in 2017 was shorter than that in 2016(P<0.001).The average cost of discharged patients in 2016 and 2017 was(21983.05±11053.83)yuan and(20262.43±10328.19)yuan respectively.The average cost of discharged patients in 2017 was lower than that in 2016(P=0.014).A total of 9444 day surgeries were performed in 2017,accounting for 11.87%of the total number of discharges.The average hospital stay for non-day surgery was(8.15±2.37)days,and the total average hospital stay for all patients was(7.30±2.09)days,which was 0.85 days shorter than the average hospital stay for non-day surgery(P=0.004).In the department of ophthalmology,plastic surgery,general surgery,otorhinolaryngology,urology,and hemangioma,the number of day operations accounted for 32.44%,20.32%,9.83%,8.32%,6.74%,and 4.95%of the total number of discharges,respectively.The average hospital stay was shortened by 1.55,1.38,0.83,0.53,0.47,and 0.35 days respectively(P<0.05).In the department of gynecology and orthopedics,day operations accounted for 4.41%and 1.21%of the total number of discharges,respectively,and had little effect on the average length of stay(P>0.05).The shortened average length of hospital stay was positively correlated with the ratio of the number of day operations to the total number of disc
